摘要
目的探讨类似卵巢性索肿瘤的子宫肿瘤的临床病理特点、诊断和鉴别诊断、治疗及预后,以提高对该病的认识。方法对1例类似卵巢性索肿瘤的子宫肿瘤的临床资料、组织病理、免疫组化进行分析,并复习相关文献。结果肿瘤细胞形成条索、小梁,条索互相吻合;细胞核小,圆形或椭圆形,核仁不明显,胞浆少,胞浆透亮或嗜酸性。CD56(-)、AE1/AE3(-)、P53(-)、EMA(-)、MelanA(-)、inhinbin(-)、calretinin(-)、CD10(-)、caldesmon(-)、CD99(+)、WT-1(+)、SMA(+)、Vim(+)、ER(+)、PR(+)、desmin(+)、Ki67(1%)。诊断:类似卵巢性索肿瘤的子宫肿瘤。结论类似卵巢性索肿瘤的子宫肿瘤无特异性临床症状,无特异性的检查可以明确诊断,需要手术后根据组织学及免疫组化确诊。其生物学行为不确定,需要长期随访。
Objective To explore the clinicopathologic features、diagnosis and differential diagnosis, treatment and prognosis of uterine tumor resembling ovarian sex cord tumor(UTROSCT). Methods One case of uterine tumor resembling ovarian sex cord tumor was analyzed with clinical data,histologic observation,immunohistochemical staining and review of the literature. Results The tumor consisted of epithelioid cells arranged in solid nests and anastomosing cords. The neoplastic cells were usually small with round to ovoid nuclei, inconspicuous nucleoli with clear or eosinophilic cytoplasm. Immunohistochemistry showed convincing positivity with WT-1, CD99, desmin, SMA, Vim, ER and PR, and the Ki67 labeling index was 1%. However, it was negative for CD56, AE1/AE3, P53, EMA, inhinbin, calretinin, CD10, MelanA and caldesmon. Conclusion The accurate diagnosis can be confirmed via histology and immunohistochemistry due to the absence of specific diagnostic imaging. UTROSCT is a rare uterine tumor which can be diagnosed with certainty on morphologic and immunohistochemical grounds. Its biologic behavior remains to be established so that close long-term follow-up is required.
