摘要
目的研究CT能谱成像应用于局灶性结节增生与肝细胞肝癌鉴别诊断中的应用效果。方法方便选取2018年3月—2019年3月期间该院接收的局灶性结节增生患者30例设为A组,将同期接收的肝癌患者30例作为B组,两组患者均给予CT能谱成像检测,对比两组患者的临床诊断结果。结果A组患者在动脉期的病变-正常实质碘浓度比(ICRLN)(t=17.831)与标准碘浓度(NIC)(t=3.644)明显高于B组﹙P<0.05﹚;A组患者的脂肪浓度明显低于B组﹙t=12.044,P<0.05﹚;两组患者在水(碘)浓度方面差异无统计学意义﹙t=1.667,P>0.05﹚;A组患者在门静期的病变-正常实质碘浓度比(ICRLN)(t=3.494)与标准碘浓度(NIC,t=20.828)明显高于B组﹙P<0.05﹚;A组患者的脂肪浓度明显低于B组﹙t=15.564,P<0.05﹚;两组患者在水(碘)浓度方面差异无统计学意义﹙t=0.235,P>0.05﹚。A、B组患者的疾病诊断准确率分别为93.33%和96.67%;A、B组的敏感性分别为96.67%和96.67%,;A、B组的特异度分别为86.67%和83.33%,差异无统计学意义﹙χ^2=0.351,0.000,0.131,P>0.05﹚。结论CT能谱成像应用于局-灶性结节增生与肝细胞肝癌患者的临床诊断与鉴别诊断中准确性较高,值得在临床中积极推广、应用。
Objective To study the application of CT energy spectrum imaging in the differential diagnosis of focal nodular hyperplasia and hepatocellular carcinoma.Methods Thirty patients with focal nodular hyperplasia received from the hospital from March 2018 to March 2019 were Conveniently selected in group A.30 patients with liver cancer received in the same period were Conveniently selected in group B.CT spectra imaging test were obtained in both groups,comparing the clinical diagnosis results of the two groups of patients.Results In the A group,the lesion-normal iodine concentration ratio(ICRLN)(t=17.831)and standard iodine concentration(NIC)(t=3.644)in the arterial phase were significantly higher than those in the B group(P<0.05)﹙t=12.044,P<0.05﹚there was no significant difference in water(iodine)concentration between the two groups﹙t=1.667,P>0.05﹚;group A patients in the portal period of the lesion-normal iodine concentration ratio(ICRLN)(t=3.494)and standard iodine concentration(NIC)(t=20.828)were significantly higher than group B﹙P<0.05﹚;group A patients had significantly lower fat concentration than group B﹙t=15.564,P<0.05﹚;The patient had no statistically significant difference in water(iodine)concentration﹙t=0.235,P>0.05﹚5.The diagnostic accuracy rates of patients in group A and group B were 93.33%and 96.67%the sensitivities of group A and group B were 96.67%and 96.67%;the specificities of group A and B were respectively 86.67%and 83.33%,the difference was not statistically significant﹙χ^2=0.351,0.000,0.131,P>0.05﹚Conclusion CT energy spectrum imaging is highly accurate in the clinical diagnosis and differential diagnosis of patients with focal nodular nodule hyperplasia and hepatocellular carcinoma.It is worthy of popularization and application in clinical practice.
