摘要
目的探讨血清betatrophin在肥胖及2型糖尿病患者循环血中的水平及其与胰岛素抵抗的关系。方法选择2016年6月~2018年3月我院新诊断的2型糖尿病(T2DM)患者83例为研究对象(观察组),同期选择我院体检健康者(NGT)者80例为对照组,按体重指数大于等于25 kg/m^2,分为正常糖调节正常体重组(NGT-NB)40例,正常糖调节肥胖组(NGT-OB)40例,2型糖尿病正常体重组(T2DM-NB)40例,2型糖尿病肥胖组43(T2DM-OB)例。测量人体指标,检测相关代谢血生化指标,采用酶联免疫吸附实验(ELISA)法测定空腹血清betatrophin水平。结果T2DM组空腹血糖(FPG)、糖化血红蛋白(HbA1c)、空腹胰岛素(FIN)、胰岛素抵抗指数(HOMA-IR)、收缩压(SBP)、舒张压(DBP)、betatrophin水平明显高于NGR组,差异有统计学意义(P均<0.05)。相关性分析显示血清betrophin与腰围(WC)、体重指数(BMI)、低密度脂蛋白(LDL-C)、总胆固醇(TC)、FPG、HbA1c、FIN、HOMA-IR呈正相关(P均<0.05)。逐步回归分析显示,LDL-C、FPG、HbA1c、HOMR-IR是血清betatrophin的独立相关因素(P均<0.05)。结论betatrophin水平可能在糖尿病及肥胖的发生、发展中发挥一定的作用。
Objective To investigate the level of serum betatrophin in normal subjects and patients with obesity and newly-diagnosed type 2 diabetes mellitus(T2 DM) and to explore the relationship of serum betatrophin with insulin resistance index(HOMA-IR). Methods From June 2016 to March 2018, 83 patients with newly diagnosed type 2 diabetes mellitus(T2 DM) and 80 normal glucose regulation(NGR) in our hospital were selected, all subjects were divided into 4 groups: NGR with normal weight group(NGR-NB), NGR with obese group(NGR-OB), T2 DM with normal weight group(T2 DM-NB), and T2 DM with obesity group(T2 DM-OB) according to their body mass index(BMI). ELISA was used to determine the level of serum betatrophin.The weight, height, waist circumference, hip circumference,and BMI,fasting insulin(FINS), lipid metabolism parameters,and insulin resistance index(HOMA-IR) were measured. Results The level of FPG, HbA1 c, fasting insulin(FINS), HOMA-IR, SBP, DBP and Betatrophin in the T2 DM group were significantly higher than that of the NGR group, and the difference was statistically significant(P<0.05). Partial correlation analysis showed that serum betatrophin was positively correlated with waist circumference, BMI, LDL-C, TC, FPG, HbA1-c, fasting insulin and HOM-IR(P<0.05).Multiple stepwise regression analysis showed that FPG, HbA1 c LDL-C and HOMR-IR were independent factors of serum Betatrophin(P<0.05). Conclusion Betatrophin level may play a role in the occurrence and development of obesity and diabetes.
