摘要
目的:探讨精索静脉曲张对精子DNA碎片、活力、形态和体外受精结局的影响。方法:选取医院2017年1月—2019年7月收治的60例精索静脉曲张所致的男性不育患者作为观察组,另选取同期的60例行IVF治疗精液正常患者作为对照组。检测两组的精子DNA碎片率、精液量、正常精子形态率、精子浓度、圆形细胞数、前向精子活力等参数;并对精子形态学及观察组的体外受精结局进行分析。结果:观察组患者的精液量、正常精子形态率、精子浓度、圆形细胞数、前向精子活力均明显低于对照组,比较差异显著(P均<0.05);精子DNA碎片率、无光晕、小光晕、中光晕、大光晕、退化精子率均明显高于对照组,比较差异显著(P均<0.05);观察组患者的体外受精率、卵裂率明显低于同期组,比较差异显著(P均<0.05)。结论:精索静脉曲张会增加男性不育患者的精子DNA碎片率,使得精子的活力、形态发生改变,降低体外受精率及卵裂率,严重影响了患者的受精结局。
Objective:To investigate the effects of varicocele on sperm DNA fragmentation,viability,morphology and in vitro fertilization outcomes.Methods:In this study,60 male patients with infertility caused by varicocele admitted from January 2017 to July 2019 were selected as the observation group,and 60 patients with normal semen treated with IVF were selected as the control group.The sperm DNA fragment rate,semen volume,normal sperm morphology rate,sperm concentration,round cell number,and forward sperm motility were measured.sperm morphology and the in vitro fertilization outcome of the observation group were analyzed.Results:The semen volume,normal sperm morphology rate,sperm concentration,round cell number,and forward sperm motility of the observation group were significantly lower than those of the control group,and the difference was significant(P<0.05);sperm DNA fragment rate,no halo,small halo,medium halo,large halo and degenerative sperm rate were significantly higher than the control group,and the difference was significant(P<0.05).The in vitro fertilization rate and cleavage rate of the observation group were significantly lower than those of the same group,the difference was significant(P<0.05).Conclusion:Varicocele increases the sperm DNA fragmentation rate of male infertile patients,which changes sperm motility and morphology,reduces in vitro fertilization rate and cleavage rate,and seriously affects the patient’s fertilization outcome.
