摘要
目的观察脑出血患者血清一氧化氮(NO)、血管内皮生长因子(VEGF)、血清低氧诱导因子-1α(HIF-1α)水平变化及其与患者临床病理特征、预后的相关性。方法选取急诊科室收治的脑出血患者51例为观察组,体检中心同期健康人群40例为对照组,分别抽取观察组入院次日及治疗后第30天、对照组体检当天的空腹静脉血测定血清NO、VEGF、HIF-1α水平,分析讨论血清NO、VEGF、HIF-1α水平与患者临床病理特征的关系,观察组治疗后第30天利用改良Rankin量表(mRS)对患者预后评分,分析血清NO、VEGF、HIF-1α水平与患者预后的关系。结果观察组患者入院次日、治疗后第30天血清中的NO、VEGF、HIF-1α水平均显著高于对照组(P<0.05),观察组患者在治疗后第30天的血清NO、VEGF、HIF-1α水平显著降低(P<0.05);Pearson相关性结果显示,血清NO、VEGF、HIF-1α水平均与患者预后mRS评分呈正相关(r=0.335、0.373、0.331,P<0.05);脑出血患者血清NO、VEGF、HIF-1α水平与其高血压史及出血量有关(P<0.05);COX多因素回归分析显示,血清NO、VEGF及HIF-1α水平高为影响患者不良预后的独立危险因素。结论观察组患者脑出血后血清NO、VEGF及HIF-1α水平相较对照组显著升高,治疗后显著下降,且与脑出血患者的预后密切相关,可能成为其预后参考的生物标志物。
Objective To observe the changes of serum nitric oxide(NO),vascular endothelial growth factor(VEGF)and hypoxia inducible factor-1α(HIF-1α)levels in patients with cerebral hemorrhage and their correlations with clinicopathological characteristics and prognosis.Methods Fifty-one patients with cerebral hemorrhage(ICH)admitted to the emergency department of our hospital from March 2016 to December 2018 were selected as the observation group,and 40 healthy people in the same period in the physical examination center of our hospital were selected as the control group.The fasting venous blood of the observation group on the day of admission and on the 30 th day after treatment,and the control group on the day of physical examination were taken to determine the levels of serum NO,VEGF and HIF-1α.The clinicopathological features of the patients was analyzed.The prognosis of the patients in the observation group was evaluated by modified Rankin scale(mRS)on the 30 th day after treatment.The correlations between serum NO,VEGF and HIF-1αlevels with prognosis of patients were also analyzed.Results The levels of serum NO,VEGF and HIF-1αin the observation group on the next day of admission and the 30 th day after treatment were significantly higher than those in the control group(P<0.05).The levels of serum NO,VEGF and HIF-1αin the observation group were significantly lower on the 30 th day after treatment(P<0.05).Pearson correlation results showed that serum NO,VEGF and HIF-1αlevels were significantly positively correlated with the prognosis of patients according to mRS score(P<0.05).The levels of serum NO,VEGF and HIF-1αin ICH patients were significantly correlated to the history of hypertension and the amount of bleeding(P<0.05).COX multivariate regression analysis showed that high levels of serum NO,VEGF and HIF-1αwere independent risk factors for adverse prognosis.Conclusion The levels of serum NO,VEGF and HIF-1αin the observation group are significantly higher than those in the control group,and significantly decrease after treatment,which is closely correlated to the prognosis of ICH patients,suggesting a biomarker for prognosis.
