超声弹性成像与常规超声诊断乳腺肿瘤良恶性的准确性特异性和敏感性研究 被引量:19

Accuracy,specificity and sensitivity of ultrasound elastography and conventional ultrasound in the diagnosis of benign and malignant breast tumors

摘要 目的探讨超声弹性成像与常规超声诊断乳腺肿瘤良恶性的准确性、特异性和敏感性。方法选取2015年8月至2019年5月期间西安高新医院收治的99例乳腺肿块患者,共计129个乳腺肿块,进行常规超声和弹性成像超声检查,对检查结果与病理结果进行对比,进行受试者工作曲线(ROC)下面积对比,评价两种检查手段在诊断乳腺肿瘤良恶性的特异性和敏感性。结果病理结果显示,良性肿块95个,恶性肿块34个。在良性乳腺肿块诊断中,常规超声诊断符合率为87.0%,弹性成像诊断符合率为90.9%;在恶性乳腺肿块诊断中,常规超声诊断符合率为60.0%,弹性成像诊断符合率为66.7%。常规超声、弹性成像诊断乳腺肿块良恶性与病理结果符合率为58.8%和84.2%,二者诊断准确率为77.5%。对ROC曲线下面积进行Z检验,结果显示,弹性成像面积大于常规超声,差异无统计学意义(Z=2.02,P=0.12),其特异度和敏感度分别为84.2%和81.1%。结论超声弹性成像能有效提升乳腺肿瘤良恶性诊断的准确度、特异性和敏感性,效果较理想,可作为早期筛查的手段。 Objective To compare the accuracy,specificity and sensitivity of ultrasound elastography and conventional ultrasound in the diagnosis of benign and malignant breast tumors.Methods A total of 99 patients with breast lumps involving 129 lumps were selected at Xi’an High-tech Hospital from August2015 to May 2019.Conventional ultrasound and ultrasound elastography were performed on all the patients.The results were analyzed and compared with the pathological results.The area under the receiver operating curve(ROC)curve(AUC)was compared and the specificity and sensitivity of the two methods in the diagnosis of benign and malignant breast tumors were evaluated.Results The results of pathological diagnosis showed that there were 95 benign lumps and 34 malignant lumps.In the diagnosis of benign breast lumps,the diagnostic coincidence rate of conventional ultrasound was 87.0%,and the diagnostic coincidence rate of ultrasound elastography was 90.9%.In the diagnosis of malignant breast lumps,the diagnostic coincidence rate of conventional ultrasound was 60.0%and the diagnostic coincidence rate of ultrasound elastography was 66.7%.The diagnostic coincidence rate was 58.8%and 84.2%,respectively for conventional ultrasound and ultrasound elastography compared with pathological diagnosis.The diagnostic accuracy was77.5%for both imaging tests,and the specificity and sensitivity was 84.2%and 81.1%,respectively.Z test of AUC showed that the area was larger for ultrasound elastography than conventional ultrasound(Z=2.02,P=0.12).Conclusion Ultrasound elastography can effectively improve the diagnostic accuracy,specificity and sensitivity for benign and malignant breast tumors.The effect is ideal and can be used as a means for early-detection screening.
