摘要 颅内动脉瘤(intracranial aneurysm,IA)系颅内动脉管壁的异常扩张或膨出,是非创伤性蛛网膜下腔出血的最常见原因。颅脑MRA具有非侵入性、无辐射、操作简单、可重复进行、费用低廉的特点;随着各种非对比增强MRA技术的发展,目前颅脑MRA在临床上已基本取代CT血管成像、DSA成为IA诊断与筛检的主要影像学检查手段。非对比4D-MRA技术除对IA进行形态学评价外,还可动态观察IA的血流动力学特征,有望对其疗效和预后进行更准确评估。本文对各种MRA技术在IA诊断的应用进展进行综述。 The intracranial aneurysm(IA)is defined as an abnormal focal dilatation of intracranial artery.The rupture of IA represents the most common cause of nontraumatic subarchnoid hemorrhage(SAH).Intracranial magnetic resonance angiography(MRA)is noninvasive,non-radiative,and characterized by simple and repeatable operation with low costs.With the rapidly development of various non-contrast enhanced MRA techniques,MRA has by far become the most popular imaging method for the diagnosis and screening of IA,and has replaced CT angiography and DSA in clinical practice to a large extent.In recent years,non-contrast enhanced 4D MRA has been applied to qualitatively and quantitatively assess the intracranial aneurysm hemodynamic alterations,and has become a promising method for evaluation of treatment efficacy and prognosis.This article reviews a variety of MRA techniques involved in the diagnosis of IA.
