摘要
目的分析脊柱泡型包虫病的CT和MRI特征及诊断价值,以提高对该病的诊断和鉴别诊断能力。方法回顾性分析经临床诊断或手术病理证实的13例脊柱泡型包虫病患者的临床和影像资料。13例行CT扫描,8例行MRI检查。结果13例脊柱泡型包虫病中,1例单椎体和12例相邻多椎体破坏,共34个椎体受累。CT扫描26个(76.5%)椎体表现为簇状或弥漫的蜂窝状、囊样和斑片样溶骨性骨质破坏。18个(52.9%)椎体破坏区周围骨质硬化,19个(55.9%)可见结节、砂砾样钙化或死骨形成。21个(61.8%)椎体附件受累,20个(58.8%)骨性终板局限性骨质破坏,13个(38.2%)椎间盘破坏,12个(35.2%)椎间隙变窄、消失。8例MRI扫描病例中,抑脂T2WI椎体及附件小囊泡检出率为62.5%(5/8)。综合CT和MRI扫描发现,在13例伴椎旁软组织泡型包虫病中,87.5%(7/8)显示高信号小囊泡。结论脊椎泡型包虫病的CT和MRI表现具有一定特征性,CT和MRI影像表现互为补充,综合分析更有利于准确诊断。
Objective To analyze the features and diagnostic value of CT and MRI scan in spinal alveolar echinococcosis and improve the diagnosis. Methods CT and MRI manifestations and clinical data of 13 cases of spinal alveolar echinococcosis confirmed by clinical diagnosis or surgical pathology were retrospectively analyzed. Thirteen patients underwent CT scan and 8 underwent MRI. Results Between 13 cases of spinal alveolar echinococcosis,1 case of single vertebral body and 12 cases of adjacent multiple vertebral bodies were destroyed,and a total of 34 vertebral bodies were involved. Twenty-six( 76. 5%) vertebral lesions showed clustered or diffuse honeycomb,cystic,and patchy osteolytic bone destruction on CT scans. Among them,Eighteen vertebral bodies( 52. 9%) had osteosclerosis around the vertebral body destruction area,19( 55. 9%) showed nodule,sand-like calcification or dead bone formation. Twenty-one( 61. 8%) vertebral attachments were involved,20( 58. 8%) had localized destruction of the vertebral endplate,13( 38. 2%) had disruption of the intervertebral disc,and 12( 35. 2%) had narrowed or disappeared intervertebral spaces. In 8 cases of MRI scan,5 cases( 62. 5%) of fat saturation T2 WI showed high-signal small vesicles in the vertebral body and attachment. Comprehensive CT scan and MRI scan showed that in 13 cases of paraspinal soft tissue,alveolar echinococcosis,87. 5%( 7/8 cases)showed high signal small vesicles. Conclusion CT and MRI images of patients with vertebral alveolar echinococcosis have certain characteristics,and CT and MRI images complement each other. Comprehensive analysis is more conducive to accurate diagnosis.
