摘要
目的评价18-23月龄儿童甲型肝炎(甲肝)减毒活疫苗(HepA-L)、麻腮风联合减毒活疫苗(MMR)单独和同时接种的免疫效果。方法采用多中心、随机、对照设计,招募无甲肝疫苗免疫史且完成首剂次含麻疹成分疫苗的18-23月龄儿童,随机分为3组,分别接种HepA-L、HepA-L与MMR、MMR,检测每名儿童免疫前、免疫后1个月血清甲肝、麻疹、腮腺炎、风疹IgG抗体,分析免疫后抗体阳性率和阳转率。结果HepA-L组儿童免疫后甲肝抗体阳性率、阳转率分别为81.3%(148/182)、80.3%(139/173)。MMR+HepA-L组儿童免疫后麻疹、风疹、腮腺炎、甲肝抗体阳性率分别为98.3%(178/181)、92.3%(167/181)、78.5%(142/181)、77.9%(147/181),阳转率分别为90.0%(9/10)、86.7%(26/30)、78.2%(129/165)、75.5%(123/163)。MMR组儿童免疫后麻疹、风疹、腮腺炎抗体阳性率分别为94.0%(157/167)、84.4%(141/167)、77.2%(129/167),阳转率分别为87.5%(14/16)、83.7%(36/43)、78.3%(123/157)。结论18-23月龄儿童单独和同时接种HepA-L和MMR均具有良好的免疫效果;同时接种对各疫苗成分的免疫效果没有影响。
Objective To evaluate the immunogenicity of hepatitis A attenuated live vaccine(HepA-L),and measles,rubella and mumps combined attenuated live vaccine(MMR)f or standalone and simultaneous vaccination in 18-23-month-old children.Methods We used a multi-center,randomized,control design to recruit 18-23-month-old children who had not received hepatitis A vaccine but had received 1 dose of measles containing vaccine,and divided them into 3 groups for vaccination with HepA-L only,HepA-L and MMR simultaneously,and MMR only.We tested subjects’serum samples to measure IgG antibody levels against hepatitis A,measles,rubella,and mumps bef ore vaccination and 1 month after vaccination to determine seropositivity rates and seroconversion rates.Results Among children in the HepA-L group,the seropositivity and seroconversion rates of hepatitis A antibody af ter vaccination were 81.3%(148/182)and 80.3%(139/173),respectively.Among children in MMR+HepA-L group,the seropositivity rates of measles,rubella,mumps and hepatitis A antibodies af ter vaccination were 98.3%(178/181),92.3%(167/181),78.5%(142/181),and 77.9%(147/181),respectively,with seroconversion rates of 90.0%(9/10),86.7%(26/30),78.2%(129/165),and 75.5%(123/163).Among children in the MMR group,the seropositivity rates of measles,rubella,and mumps antibodies af ter vaccination were 94.0%(157/167),84.4%(141/167),and 77.2%(129/167),respectively,with seroconversion rates of 87.5%(14/16),83.7%(36/43),and 78.3%(123/157).Conclusions Both standalone and simultaneous administration of HepA-L and MMR vaccines showed good immunogenicity among 18-23-month-old children.Simultaneous vaccination did not affect immunogenicity of the vaccine antigens.
