摘要
目的探讨老年人输尿管结石在输尿管镜气压弹道碎石术后应用坦索罗辛临床疗效评价。方法选取2017年9月至2019年3月在朝阳市中心医院泌尿外科收治的78例老年输尿管结石患者作为此次研究对象,按照随机数表法分为治疗组(坦索罗辛联合气压弹道碎石组)39例和对照组(单纯气压弹道碎石组)39例。治疗按照常规行气压弹道波碎石治疗,术后联合探索罗新(0.4 mg,每晚1次,共4 w)辅助治疗,对照组单纯行输尿管镜下气压弹道碎石治疗,比较两组患者各项临床资料,进行评价。结果治疗组术后4 w结石残余率5.1%(2/39)要低于对照组23.1%(9/39);治疗组术后镇痛药(双氯芬酸钠栓剂)使用率及腰腹痛发生率25.6%(10/39)、25.6%(10/39)要低于对照组51.3%(20/39)、56.4%(22/39),差异有统计学意义(P<0.05)。结论对于老年输尿管结石患者,输尿管镜气压弹道碎石术后联合运用坦索罗辛可提高清石率、降低术后镇痛药的使用率及腰腹痛的发生率,提高了老年输尿管结石患者生活质量,适用于老年输尿管结石的临床治疗,值得推广。
Objective To evaluate the clinical efficacy of tansulosin in the treatment of ureteral calculi in elderly patients after ureteroscopic pneumatic lithotripsy.Methods 78 elderly patients with ureteral calculi admitted to the urology department of chaoyang central hospital from September 2017 to March 2019 were selected as the subjects of the study.According to the random number table method,39 patients formed the treatment group(tansulosin combined with pneumatic lithotripsy group)and 39 patients formed the control group(pneumatic lithotripsy group).The patients in the treatment group were treated with pneumatic ballistic wave lithotripsy as the conventional treatment,combined with tansulosin(0.4 mg,once per night for 4 w)as adjuvant therapy after the operation,and the control group was treated simply with pneumatic lithotripsy.The clinical data of the two groups were compared and evaluated.Results Four weeks after surgery,the stone residual rate was 5.1%(2/39)in the treatment group which was lower than 23.1%(9/39)in the control group.The utilization rate of postoperative analgesics(diclofenac sodium suppository)and the incidence of lumbago and abdominal pain in the treatment group were 25.6%(10/39)and 25.6%(10/39)which were lower than those in the control group,51.3%(20/39)and 56.4%(22/39),with statistically significant differences(P<0.05).Conclusion For elderly patients with ureteral calculi,combined application of tansulosin after ureteroscope pneumatic lithotripsy can improve the stone removal rate,reduce the utilization rate of postoperative analgesics and the incidence of lumbar and abdominal pain,and improve the quality of life,which is suitable for the clinical treatment of elderly patients with ureteral calculi and is worth popularizing.
