摘要
目的:探究鼻内镜手术联合布地奈德喷鼻治疗慢性鼻窦炎-鼻息肉的效果。方法:选取2017年1月-2018年5月于笔者所在医院拟行鼻内镜手术的124例慢性鼻窦炎-鼻息肉患者为研究对象,采用随机数字表法将其分为研究组与对照组各62例。对照组术后仅应用生理盐水冲洗鼻腔,研究组应用布地奈德喷鼻剂。比较两组临床疗效、复发率、术前及术后1、4、12、24周鼻气道阻力及嗅觉功能,以及治疗前及治疗12周后鼻窦CT评分及鼻内镜评分。结果:研究组治疗总有效率明显高于对照组,复发率明显低于对照组,差异均有统计学意义(P<0.05)。术后1、4、12、24周,两组鼻气道阻力及嗅觉功能评分均较术前显著下降,且研究组均明显低于对照组,差异有统计学意义(P<0.05)。治疗12周后,两组患者鼻窦CT评分及鼻内镜评分均较治疗前显著下降,且研究组均明显低于对照组,差异有统计学意义(P<0.05)。结论:鼻内镜手术联合布地奈德治疗慢性鼻窦炎-鼻息肉效果显著,能够有效改善鼻气道阻力及嗅觉功能,利于患者预后。
Objective:To explore the effect of nasal endoscopic surgery combined with Budesonide Nasal Spray in the treatment of chronic sinusitis and nasal polyps.Method:A total of 124 patients with chronic sinusitis and nasal polyps who were scheduled to undergo nasal endoscopic surgery in our hospital from January 2017 to May 2018 were selected as the subjects.They were divided into the study group and the control group by random number table method,62 cases in each group.The control group only used normal saline to rinse nasal cavity,while the study group used Budesonide Nasal Spray.The curative effect,recurrence rate,nasal airway resistance and olfactory function at 1,4,12 and 24 weeks before and after surgery,as well as nasal sinus CT score and nasal endoscopic score before and 12 weeks after treatment were compared between the two groups.Result:The total effective rate of the study group was significantly higher than that of the control group,and the recurrence rate was significantly lower than that of the control group,the differences were statistically significant(P<0.05).At 1,4,12 and 24 weeks after surgery,the nasal airway resistance and olfactory function scores in both groups were significantly lower than those before surgery,and the study group were significantly lower than those of the control group,the differences were statistically significant(P<0.05).After 12 weeks of treatment,the nasal sinus CT and nasal endoscopic score in both groups were significantly lower than those before treatment,and the study group were significantly lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion:Nasal endoscopic surgery combined with Budesonide has a significant effect in the treatment of chronic sinusitis and nasal polyps,and can effectively improve nasal airway resistance and olfactory function,which is beneficial to the prognosis of patients.
