摘要
目的:探讨重症肺炎继发呼吸衰竭患者应用无创机械通气对患者的效果及对炎症因子的影响。方法:选取2017年1月-2019年9月笔者所在医院收治的86例重症肺炎继发呼吸衰竭患者作为研究对象,使用随机数字法分为对照组与观察组,各43例。予以对照组有创机械通气,给予观察组无创机械通气。对比分析两组疗效及炎症因子水平变化情况。结果:观察组总有效率明显优于对照组(P<0.05);治疗后,观察组IL-4、CRP、TNF-α均明显低于对照组(P<0.05);两组治疗前HR、RR、pH值、PaO2和PaCO2检测结果比较差异无统计学意义(P>0.05);治疗后观察组PaO2高于对照组,PaCO2低于对照组,数据比较差异有统计学意义(P<0.05)。结论:对重症肺炎继发呼吸衰竭患者采用无创机械通气,能够有效降低炎症因子水平,改善血气指标,提高肺功能,临床疗效显著,值得推广。
Objective:To investigate the effect of non-invasive mechanical ventilation on patients with respiratory failure secondary to severe pneumonia and the influence of inflammatory factors.Method:A total of 86 cases of secondary respiratory failure to severe pneumonia admitted to our hospital from January 2017 to September 2019 were selected as the study subjects,and randomly divided into the control group and the observation group,43 cases in each group.Invasive mechanical ventilation was given to the control group and non-invasive mechanical ventilation was given to the observation group.The curative effect and the change of inflammatory factor level between the two groups were compared.Result:The total effect rate of the observation group was significantly better than that of the control group(P<0.05).After treatment,IL-4,CRP and TNF-αin the observation group were significantly lower than those in the control group(P<0.05).There was no significant difference in the results of HR,RR,pH,PaO2 and PaCO2 between the two groups before treatment(P>0.05).After treatment,PaO2 in the observation group was higher than that in the control group,and PaCO2 was lower than that in the control group,with statistically significant difference(P<0.05).Conclusion:Non-invasive mechanical ventilation can effectively reduce the level of inflammatory factors,improve blood gas index and improve lung function in patients with secondary respiratory failure due to severe pneumonia.
