摘要
目的:利用培养管硅胶显色法直接检测临床涂阳痰标本中结核分枝杆菌(MTB)吡嗪酰胺(PZA)药物敏感性,并对该方法进行临床应用评价。方法:对2016年6月-2017年10月在南通市第六人民医院就诊的158例肺结核患者涂阳痰标本进行预处理,利用Bactec MGIT960与培养管硅胶显色法同步进行PZA药物敏感性检测,并对检测结果进行比较。结果:以Bactec MGIT960检测PZA药物敏感性结果为判断标准,培养管硅胶显色法灵敏度、特异性、符合率分别为96.8%、95.9%、96.7%。报告结果平均时间为(24.5±2.0)d,同Bactec MGIT960检测时间相仿,报告时间与痰中MTB菌量及活力有相关性。结论:培养管硅胶显色法可以替代Bactec MGIT960直接对临床涂阳痰标本进行PZA药物敏感性检测,结果可靠,操作简便,成本低廉,满足临床快速诊断的需要,适合推广使用。
Objective:Using Culture tube silica gel assay to directly detect Pyrazinamide(PZA)drug sensitivity of mycobacterium tuberculosis(MTB)in clinical specimens of sputum smear-positive,and to evaluate clinical application of this method.Method:A total of 158 cases sputum smear-positive samples of pulmonary tuberculosis patients from June 2016 to October 2017 in Nantong Sixth People’s Hospital were pretreatmented,PZA drug sensitivity test were synchronous detected by Bactec MGIT960 and culture tube silica gel assay,and the test results were compared.Result:The results of PZA drug sensitivity by Bactec MGIT960 as the judgment standard,sensitivity,specificity and accuracy of culture tube silica gel assay were 96.8%,95.9%and 96.7%respectively.The average time of report results was for(24.5±2.0)d,similar with Bactec MGIT960 testing time,report time were correlationsand with the number of MTB in sputum and dynamic.Conclusion:The culture tube silica gel assay can replace Bactec MGIT960 for directly detecting PZA resistance of MTB in smear-positive sputum,the result is reliable,simple operation,low cost.It can meet the need of rapid clinical diagnosis and worthy in application.
