苦参碱注射液对中晚期口腔癌患者炎症、凋亡及侵袭转移相关因子表达的影响 被引量:3

摘要 目的观察苦参碱注射液联合化疗对中晚期口腔癌患者炎症、凋亡及侵袭转移相关因子表达的影响。方法选取2016年1月至2018年10月接受治疗的中晚期口腔癌患者60例,随机分为对照组和治疗组,每组30例。对照组给予常规化疗方案,治疗组在常规化疗基础上加用苦参碱注射液。疗程均为12周。酶联免疫吸附法(ELISA)测定2组治疗前后血清炎症、凋亡、侵袭转移和TLRs/NF-κB信号通路相关因子水平。流式细胞仪测定2组治疗前后外周血单核细胞(PBMCs)中TLR2、TLR4、NF-κB阳性细胞数。结果治疗前2组血清IL-6、TNF-α、Bax、Bcl-2、VEGF、MMP-9、TLR2、TLR4、NF-κB水平、PBMCs中TLR2、TLR4、NF-κB阳性细胞数比较差异无统计学意义(P>0.05)。治疗后2组血清IL-6、TNF-α、Bcl-2、VEGF、MMP-9、TLR2、TLR4、NF-κB水平、PBMCs中TLR2、TLR4、NF-κB阳性细胞数均明显低于治疗前,血清Bax水平均明显高于治疗前(P<0.05),且治疗组血清IL-6、TNF-α、Bcl-2、VEGF、MMP-9、TLR2、TLR4、NF-κB水平、PBMCs中TLR2、TLR4、NF-κB阳性细胞数低于对照组,血清Bax水平高于对照组(P<0.05)。结论苦参碱注射液联合化疗治疗中晚期口腔癌疗效确切,能够显著抑制炎性反应和血管生成,调节凋亡相关因子表达,其机制与抑制TLRs/NF-κB信号通路的激活及下游炎症、凋亡、侵袭转移相关因子表达有关。 Objective To observe the effects of matrine injection combined with chemotherapy on the expression of inflammation,apoptosis and invasion and metastasis-related factors in patients with advanced oral cavity carcinoma(OCC).Methods A total of 60 patients with advanced OCC who were treated from January 2016 to October 2018 were randomly divided into the control group(n=30)and the treatment group(n=30).The control group was given a conventional chemotherapy regimen,and the treatment group was given matrine injection on the basis of conventional chemotherapy.The course of treatment was 12 weeks.Enzyme-linked immunosorbent assay(ELISA)was used to determine serum inflammation,apoptosis,invasion and metastasis,and TLRs/NF-κB signaling pathway-related factors before and after treatment in the two groups.Flow cytometry was used to determine the number of TLR2,TLR4,and NF-κB positive cells in peripheral blood mononuclear cells(PBMCs)before and after treatment in the two groups.Results Before treatment there were no significant differences in the levels of IL-6,TNF-α,Bax,Bcl-2,VEGF,MMP-9,TLR2,TLR4,NF-κB,and the number of TLR2,TLR4,and NF-κB positive cells in PBMCs between the two groups(P>0.05).After treatment,the serum levels of IL-6,TNF-α,Bcl-2,VEGF,MMP-9,TLR2,TLR4,NF-κB,and the number of TLR2,TLR4,and NF-κB positive cells in PBMCs were significantly lower than those before treatment,while the serum Bax levels were significantly higher than those before treatment(P<0.05).The serum levels of IL-6,TNF-α,Bcl-2,VEGF,MMP-9,TLR2,TLR4,NF-κB,and the number of TLR2,TLR4,and NF-κB positive cells in PBMCs in treatment group were significantly lower than those in control group,whereas the serum Bax levels were significantly higher than those in control group(P<0.05).Conclusion Matrine injection combined with chemotherapy is effective in treating advanced OCC,which can significantly inhibit inflammatory response and angiogenesis and regulate the expression of apoptosis-related factors,and its action mechanism is associated with inhibition of TLRs/NF-κB signaling pathway activation and expressions of downstream inflammation,apoptosis,invasion,and metastasis related factors.
