摘要
目的探讨促甲状腺激素(TSH)、抗甲状腺过氧化物酶抗体(TpoAb)、抗甲状腺球蛋白抗体(TgAb)检测对甲状腺疾病的临床诊断价值。方法将2017年3月-2019年1月在佳木斯市中心医院接受治疗的200例甲状腺疾病患者组成观察组,依据病因不同分为桥本氏组(50例)、毒性弥漫性甲状腺肿(Graves)组(45例)、甲亢组(45例)、甲状腺功能减退组(60例),同时选取同期在我院接受健康体检的50例志愿者组成对照组,分别检测TSH、TpoAb和TgAb水平,分析检测结果。结果桥本氏组、Graves组、甲亢组TSH水平低于对照组,TpoAb、TgAb水平均高于对照组,甲状腺功能减退组TSH、TpoAb、TgAb水平均高于对照组,差异有统计学意义(P<0.05);桥本氏组、Graves组、甲亢组、甲状腺功能减退组TSH、TpoAb、TgAb阳性率均比对照组高,其中桥本氏组和Graves组的TpoAb、TgAb阳性率均比甲亢组和甲状腺功能减退组高,TSH阳性率均比甲亢组和甲状腺功能减退组低,差异有统计学意义(P<0.05)。结论 TSH、TpoAb、TgAb检测对甲状腺疾病均有一定诊断价值,且可有效鉴别不同种类甲状腺疾病类型。
Objective To investigate the clinical diagnostic value of thyrotropin(TSH), anti-thyroid peroxide antibody(TpoAb)and anti-thyroglobulin antibody(TgAb) in the diagnosis of thyroid diseases. Methods 200 patients with thyroid diseases treated in the hospital from March, 2017 to January, 2019 were divided into three groups according to their etiological factors: Hashimoto group(n=50), toxic disseminated goiter(Graves) group(n=45), hyperthyroidism group(n=45) and hypothyroidism group(n=60). At the same time, 50 cases received physical examination in the hospital at the same time. The levels of TSH, TpoAb and TgAb were measured in the control group,and the results were analyzed. Results The levels of TSH, TpoAb, TgAb, TSH and TpoAb, TgAb in Hashimoto group, Graves group and hyperthyroidism group were lower than those in control group, and the positive rates of TSH,TpoAb, TgAb in Hashimoto group, Graves group, hyperthyroidism group and hypothyroidism group were significantly higher than those in control group(P<0.05). The positive rates of TSH, TpoAb and TgAb in Hashimoto group, Graves group, hyperthyroidism group and hypothyroidism group were higher than those in control group. The positive rate of TpoAb and TgAb in Graves group was higher than that in hyperthyroidism group and hypothyroidism group, and the positive rate of TSH was lower than that in hyperthyroidism group and hypothyroidism group. The difference was statistically significant(P<0.05). Conclusion TSH, TpoAb and TgAb detection has certain diagnostic value in thyroid diseases, and can effectively distinguish different kinds of thyroid diseases.
