摘要
目的 探讨急诊抢救急性有机磷农药中毒合并呼吸衰竭的效果。方法 随机抽取2015年1月至2019年1月晋中市第二人民医院收治的急性有机磷农药中毒合并呼吸衰竭患者70例,男32例,女38例,年龄18~87岁。70例患者均给予针对性的急诊抢救措施,回顾性分析其抢救效果及抢救前后相关指标的变化。结果 70例有机磷农药中毒者经抢救病情好转68例(97.14%,68/70),死亡1例(1.43%,1/70),放弃治疗1例(1.43%,1/70)。68例急诊抢救成功者,抢救后其呼气峰流速、每分钟通气量、血氧分压、酸碱度均较抢救前增高,而二氧化碳分压则较抢救前下降(P<0.05)。结论 对急性有机磷农药中毒合并呼吸衰竭患者予以洗胃、气管插管机械通气、对症药物等综合救治,能改善血氧指标,有利于提高抢救成功率,降低病死率。
Objective To investigate the effects of emergency rescue on acute organophosphorus pesticide poisoning combined with respiratory failure.Methods Seventy patients with acute organophosphorus pesticide poisoning complicated with respiratory failure treated in Jinzhong Second People’s Hospital from January 2015 to January 2019 were selected.There were 32 males and 38 females,aged 18-87 years old.Seventy patients were given targeted emergency rescue measures,and their rescue effects and changes in related indicators before and after rescue were retrospectively analyzed.Results Of the 70 patients with organophosphorus pesticide poisoning,68 cases(97.14%,68/70)were cured,1 case died(1.43%,1/70),and 1 case(1.43%,1/70)got withdrawal of treatment for the relationss’decision.Among the 68 patients who were successfully rescued by emergency,the peak expiratory flow rate,minute ventilation,blood oxygen partial pressure and pH were higher than those before rescue,and the partial pressure of carbon dioxide decreased after rescue(P<0.05).Conclusions Comprehensive treatment of acute organophosphorus pesticide poisoning combined with respiratory failure in patients,including gastric lavage,tracheal intubation mechanical ventilation and symptomatic drugs,can improve blood oxygenation index,improve rescue success rate and reduce mortality,which is conducive to improving the success rate of rescue and reducing the mortality rate.
