摘要
目的:探究经腹腔入路腹腔镜肾上腺切除术治疗肾上腺肿瘤临床观察。方法:选取2017年5月-2019年5月在我院接受治疗的64例肾上腺肿瘤患者,按随机数字表法将其分成2组,对照组32例患者采用后腹腔镜下肾上腺切除术治疗;观察组32例患者采用腹腔入路腹腔镜肾上腺切除术治疗。对比两组患者手术前后血清胱抑素C(Cys C)、肌酐(sCr)、β2微球蛋白(β2-MG)、血清尿素氮(BUN)、C蛋白反应(CRP)、白细胞(WBC)、中性粒细胞比率(NEU)、白细胞介素-6(IL-6)、谷胱甘肽过氧化物酶(GSH-Px)、超氧化物歧化酶(SOD)、晚期氧化蛋白产物(AOPP)、血浆丙二醛(MAD)的变化情况。结果:观察组手术后Cys C、sCr、β2-MG、BUN显著低于对照组(P<0.05);观察组手术后CRP显著高于对照组,而WBC、NEU、IL-6则显著低于对照组(P<0.05);观察组手术后GSH-Px、SOD、AOPP、MAD水平显著低于对照组(P<0.05)。结论:经腹腔入路腹腔镜肾上腺切除术治疗肾上腺肿瘤临床效果明显,能有效改善肾功能及各实验室相关指标,抑制炎性因子。
Objective: To investigate the clinical observation of laparoscopic adrenalectomy via celiac approach in the treatment of adrenal tumors. Methods: from May 2017 to may 2019,64 patients with adrenal tumor were divided into two groups according to the random number table. 32 patients in the control group were treated with retroperitoneal laparoscopic adrenalectomy,and 32 patients in the observation group were treated with celiac laparoscopic suprarenal adenoidectomy. Serum cystatin C( Cys C),creatinine( SCR),β2-microglobulin( β2-mg),serum urea nitrogen( BUN),C-protein response( CRP),leukocyte( WBC),neutrophil ratio( neu),interleukin-6( IL-6),glutathione peroxidase( GSH PX),superoxide dismutase( SOD),advanced oxidation protein product( AO)were compared before and after operation The changes of plasma malondialdehyde( MAD). Results: the levels of Cys C,SCR,β2-mg and BUN in the observation group were significantly lower than those in the control group( P<0.05);the levels of CRP,WBC,NEU and IL-6 in the observation group were significantly lower than those in the control group( P< 0.05);the levels of GSH Px,SOD,AOPP and mad in the observation group were significantly lower than those in the control group( P<0.05). Conclusion: laparoscopic adrenalectomy via celiac approach is effective in the treatment of adrenal tumors. It can effectively improve renal function and related laboratory indicators,and inhibit inflammatory factors.
