248株碳青霉烯类耐药肠杆菌科细菌临床特点及耐药性分析 被引量:25

摘要 目的了解本院耐碳青霉烯类肠杆菌科细菌(CRE)的分布及耐药情况,为临床预防和治疗CRE感染提供参考依据。方法收集北京中医药大学东直门医院2016年1月-2018年12月间从临床标本中分离的CRE,采用美国BD公司的PHOENIX-100全自动微生物鉴定药敏系统进行鉴定及药物敏感性试验,采用改良Hodge试验对碳青霉烯酶产酶情况进行检测。结果共收集248株CRE,主要为肺炎克雷伯菌,其次为大肠埃希菌和阴沟肠杆菌,其标本来源主要为痰液,其次为尿液和脓液及分泌物,科室分布主要为重症监护室(ICU),其次为脑病科和呼吸科。结论CRE的耐药现象比较严重,主要分布于重症监护病房,临床应加强对医院感染的预防和控制,防止耐药菌感染的爆发。 Objective To investigate the distribution and resistance of Carbapenem-resistant Enterobacteriaceae,and to provide reference for the prevention and treatment of CRE infection.Methods To collect the CRE separated from clinical specimens by Dongzhimen Hospital of Beijing University of Traditional Chinese Medicine from January 2016 to December 2018,and use the PHOENIX-100 to identify and test the drug sensitivity test.The improved Hodge test was used to detect the enzyme production status of carbene enzyme.Results A total of 248 CREs were collected,mainly Klebsiella Pneumoniae,followed by Escherichia coli and Enterobacter cloacae.The source of the specimens was mainly sputum,followed by urine and pus and secretions.The department was mainly distributed in the intensive care unit(ICU),followed by the neurology department and the respiratory department.Conclusion The drug resistance of CRE is serious and is mainly distributed in the intensive care unit.Clinical prevention and control of hospital infection should be strengthened to prevent the outbreak of drug-resistant infections.
