摘要
经皮椎体强化术(PVA)主要包括经皮椎体成形术(PVP)及经皮椎体后凸成形术(PKP),它是目前骨质疏松性压缩骨折及椎体肿瘤等疾病的主要外科治疗方式,有微创、有效缓解疼痛及明显改善患者生活质量等优点。骨水泥椎间盘渗漏(CDL)是PVA的常见并发症,既往多数研究认为CDL加速椎间盘退行性变(IVDD)、增加邻近椎体骨折(AVF)风险并导致二次甚至多次手术。然而这个结论也并非毫无争议,当前也有不少研究认为CDL并不增加AVF发生风险。本文将就近年来关于CDL的研究进展做一综述以期提高大家对它的认识。
Percutaneous vertebral augmentation(PVA)-a minimally invasive surgery for osteoporotic compression fractures,neoplastic vertebra and other painful vertebras,mainly includes percutaneous vertebroplasty(PVP)and percutaneous kypho⁃plasty(PKP).PVA has the advantages of minimally invasive trauma,effectively relieving the pain and improving quality of life of the patients.Bone cement disc leakage(CDL)is a common complication of PVA and many previous studies have suggested that CDL accelerates disc degeneration(IVDD),increases the risk of adjacent vertebrae fracture(AVF),and leads to reopera⁃tion or even repeated operation.However,some of recent studies have concluded that CDL does not increase the risk of AVF.In this article the advances of CDL in recent years were reviewed in order to improve the understanding about it.
