摘要
目的探讨外周血CD4+CD25+Treg细胞与老年动脉粥样硬化性脑梗死(CI)颈动脉内中膜厚度(CIMT)及预后的相关性。方法选取2016年5月~2017年12月江南大学附属医院收治的80例老年动脉粥样硬化性CI患者作为观察组,同期因胸闷就诊但冠脉造影正常的72例老年患者作为对照组。检测两组患者外周血CD4+CD25+Treg细胞百分比,并分析其与脑血管病危险因素、CIMT及预后相关性。结果 CD4+CD25+Treg细胞百分比是引发CI的独立因素;观察组外周血CD4+CD25+Treg细胞百分比明显低于对照组(P <0.05);外周血CD4+CD25+Treg细胞百分比与CIMT呈显著负相关(P <0.001),其预测老年动脉粥样硬化性CI患者预后的敏感性为82.6%、特异性为67.8%。结论外周血CD4+CD25+Treg细胞百分比可作为CI的独立影响因素,与CIMT呈负相关,对于CI的严重程度及预后具有较高的预测效能。
Objective To investigate the correlation between peripheral blood CD4+CD25+Treg cells and carotid intima-media thickness(CIMT) and prognosis in older patients with atherosclerotic cerebral infarction.Methods 80 older patients with atherosclerotic cerebral infarction were selected as the observation group in the affiliated hospital to Jiangnan University from May 2016 to December 2017.72 older patients with chest distress but normal coronary angiography were selected as the control group.The percentage of CD4+CD25+Treg cells in peripheral blood was measured.The relationship between the percentage of CD4+CD25+Treg cells and risk factors of cerebrovascular disease,CIMT and clinical prognosis was analyzed.Results The percentage of CD4+CD25+Treg cells was an independent factor for cerebral infarction.The percentage of CD4+CD25+Treg in the observation group was lower than that in the control group(P < 0.05).The percentage of CD4+CD25++ Treg was negatively correlated with CIMT(P <0.001).The sensitivity and specificity of percentage of peripheral blood CD4+CD25+Treg cells to predict the prognosis of older patients with atherosclerotic cerebral infarction were 82.6% and 67.8%,respectively.Conclusion The percentage of CD4+CD25+Treg cells in peripheral blood can be used as a sensitive indicator of the severity of atherosclerosis and has a high predictive power for the prognosis in older patients with atherosclerotic cerebral infarction.
