摘要
目的探讨气压喷砂磨光技术对种植体周围炎的洁治效果。方法纳入已植入种植义齿并且时间>半年的种植体周围炎患者,根据患者意愿分为对照组与试验组,对照组主要通过生理盐水漱口方式清洗种植体,试验组采用气压喷砂磨光技术对种植体进行洁治,洁治1次/w。连续洁治8 w后,比较两组种植体周围组织的牙周指数,包括临床附着水平(CAL)、牙龈指数(GI)、改良菌斑指数(mPLI)、龈沟出血指数(SBI)、牙龈探诊深度(PD),以及种植体龈沟液中的白细胞介素1α、1β(IL-1α、IL-1β)的浓度变化。结果洁治后,试验组牙周指数显著低于对照组(P<0.05),并且龈沟液IL-1α、IL-1β浓度也显著低于对照组(P<0.05)。结论气压喷砂磨光治疗方法可以显著改善种植体的牙周指数,降低龈沟液IL-1α、IL-1β浓度,进而达到治疗种植体周围炎的效果,值得临床推广应用。
Objective To explore the scaling effect of air-abrasive polishing technique on peri-implantitis.Methods The patients with peri-implantitis who have denture implanted for over six months were included and divided into a control group and an experimental group according to their willingness.In the control group,implants were cleaned by gargling with normal saline;in the experimental group,the implants were scaled with air-abrasive polishing technique once a week.After 8 w continuous scaling,comparison was made between the two groups regarding the periodontal indexes of peri-implant tissues,including clinical attachment level(CAL),gingival index(GI),modified plaque index(mPLI),gingival sulcus bleeding index(SBI),gingival probing depth(PD),and changes of concentrations of interleukin 1α,1β(IL-1α,IL-1β)in gingival crevicular fluid of implants.Results After the scaling,the values of periodontal indexes in the experimental group were significantly lower than that in the control group(P<0.05),and the concentrations of IL-1αand IL-1βwere significantly lower as well(P<0.05).Conclusion The therapeutic method of air-abrasive polishing can significantly improve the values of periodontal indexes of implants and reduce the concentrations of IL-1αand IL-1βin gingival crevicular fluid,thus achieving the therapeutic effect for peri-implantitis,which deserves clinical application.
