摘要
【目的】观察浮针疗法治疗气滞血瘀型腰椎间盘突出症(LDH)的安全性和有效性。【方法】将70例气滞血瘀型腰椎间盘突出症患者随机分为治疗组和对照组,每组各35例。2组患者均给予口服活血止痛软胶囊的基础治疗,治疗组同时给予肌筋膜疼痛触发点(MTrP)处的浮针治疗,对照组同时给予传统针灸治疗,疗程为2周。观察2组患者治疗前及治疗后1周、2周、1个月和3个月的疼痛视觉模拟量表(VAS)评分、Oswestry功能障碍指数(ODI)评分和日本骨科医师协会(JOA)神经功能评分的变化情况,评价2组治疗的安全性。【结果】(1)治疗后各时间点,2组患者的VAS评分和ODI评分均较治疗前明显降低(P<0.05),JOA评分均较治疗前明显升高(P<0.05),且治疗组各时间点的VAS评分和ODI评分均明显低于对照组,JOA评分均明显高于对照组,差异均有统计学意义(P<0.05)。(2)治疗过程中,2组患者均未出现晕针、断针、滞针、皮下出血或血肿等严重不良事件。【结论】浮针疗法与传统针灸疗法治疗气滞血瘀型腰椎间盘突出症均有一定的治疗效果,但浮针疗效优于传统针灸疗法。
Objective To observe the clinical efficacy and safety of floating acupuncture therapy for the treatment of lumbar disc herniation(LDH)of qi stagnation and blood stasis type. Methods Seventy patients with lumbar intervertebral disc herniation of qi stagnation and blood stasis type were divided into trial group and control group,35 cases in each group. The two groups were given fundamental treatment with Huoxue Zhitong Soft Capsules orally,and additionally,the trial group was given floating acupuncture therapy on the myofascial trigger points(MTrP),and the control group was given conventional acupuncture. The course of the treatment for the two groups covered 2 weeks. Before treatment,and one week,2 weeks,one month,and 3 months after treatment,the scores of pain visual analogue scale(VAS),Oswestry Dability Index(ODI)and Japanese Orthorpedics Association(JOA)for neurological function in the two groups were observed. After treatment,the safety was evaluted. Results(1)At various time points after treatment, the VAS scores and ODI scores of the two groups were markedly decreased and JOA scores were obviously increased in comparison with those before treatment(P<0.05),and the trial group had lower VAS scores and ODI scores,and had higher JOA scores than the control group at various time points,the difference being statistically signficant(P<0.05).(2)During the trial,no severe adverse events such as faint during acupuncture,breaking of the needle,sticking of needle,subcutaneous hemorrhage or hematoma were found in the two groups. Conclusion Both floating acupuncture therapy and conventional acupuncture therapy have certain therapeutic effect on lumbar intervertebral disc herniation of qi stagnation and blood stasis type,and the curative effect of floating acupuncture is better than conventional acupuncture.
