摘要
目的分析评价在超声引导下进行髂腹股沟-髂腹下神经阻滞复合局部浸润麻醉在老年患者腹股沟疝修补术中的价值。方法选取2018年5~12月于我院普外科治疗的55例行疝气修补术的年龄> 60岁的患者为研究对象,采用随机数字表法分为实验组和对照组,实验组26例,对照组29例,均给予局部浸润麻醉,实验组联合进行超声引导下髂腹股沟-髂腹下神经阻滞,比较两组麻醉效果、麻醉时间、疼痛视觉模拟评分(VAS)、术中血流动力学情况、并发症发生情况。结果实验组麻醉效果显著优于对照组(P <0.05),实验组麻醉起效时间、持续时间显著优于对照组(P <0.05),实验组疝囊牵拉时VAS评分显著低于对照组(P <0.05),实验组术中血压、心率、SPO2均显著优于对照组(P<0.05),两组并发症比较,差异无统计学意义(P> 0.05)。结论超声引导下进行髂腹股沟-髂腹下神经阻滞复合局部浸润麻醉在老年患者腹股沟疝修补术中有显著疗效,适用于临床。
Objective To analyze and evaluate values of ilioinguinal iliac inguinal-inferior iliac nerve block combined with local infiltration anesthesia under ultrasound guidance in inguinal hernia repair in elderly patients. Methods 55 patients over 60 years old who underwent hernia repair in department of general surgery in our hospital from May to December 2018 were selected as research objects. According to the random number table method,they were divided into the experimental group with 26 cases and the control group with 29 cases.They were all given local infiltration anesthesia.The experimental group was also given ilioinguinal iliac inguinal-inferior iliac nerve block under ultrasound guidance.The anesthetic effect,anesthesia time,visual analogue scale(VAS),intraoperative hemodynamics and complications were compared between the two groups. Results The anesthesia effect of the experimental group was significantly better than that of the control group(P < 0.05).The onset time and duration of anesthesia in the experimental group were significantly better than those in the control group(P < 0.05).The VAS score of hernia sacs in the experimental group was significantly lower than that in the control group(P < 0.05).The blood pressure,heart rate,and SPO2 of the experimental group were significantly better than those of the control group(P < 0.05).There were no significant differences in complications between the two groups(P > 0.05). Conclusion Ilioinguinal iliac inguinal-inferior iliac nerve block combined with local infiltration anesthesia under ultrasound guidance in inguinal hernia repair in elderly patients has significant curative effects,which is suitable for clinical practice.
