激光辅助孵化对冻融胚胎移植临床结局的影响 被引量:1

摘要 目的:探讨激光辅助孵化在<38岁患者冻融卵裂期胚胎移植中的应用效果。方法:回顾性分析接受体外受精-胚胎移植(IVF-ET)失败后首次行冻融胚胎移植(FET)的<38岁患者的临床资料。解冻移植周期未辅助孵化的患者纳入对照组,解冻移植周期行辅助孵化的患者纳入辅助孵化组。比较2组优质胚胎占比、复苏率、临床妊娠率、着床率、多胎妊娠率、异位妊娠率等妊娠情况指标。结果:2组的女方平均年龄、体质量指数、不孕时间、原发不孕占比、子宫内膜准备方案、多胎妊娠率和异位妊娠率差异均无统计学意义(P>0.05);与对照组比较,辅助孵化组临床妊娠率(76.62%vs 33.33%)和着床率(53.57%vs 22.62%)均分别高于对照组,差异有统计学意义(P<0.05);结论:激光辅助孵化可提高冻融卵裂期胚胎移植的临床妊娠率和着床率,不增加多胎妊娠和异位妊娠的风险。 Objective:To investigate the effect of laser assisted hatching in freeze-thaw cleavage embryo transfer in patients<38 years old.Methods:Clinical data of patients less than 38 years old who underwent freeze-thaw embryo transfer(FET)for the first time after failure of in vitro fertilization-embryo transfer(IVF-ET)in our center were retrospectively analyzed.Patients who did not underwent hatching during freeze-thaw embryo transfer from January 2016 to November 2017(before November 2017,assisted hatching technology had not been carried out in our center)were included in the control group.From November 2017 to June 2019(after November 2017 our center carry out assisted hatching technology)patients who underwent hatching during freeze-thaw embryo transfer were included in assisted hatching group.Comparison of pregnancy indicators such as the proportion of high-quality embryos,resuscitation rate,clinical pregnancy rate,implantation rate,multiple pregnancy rate,and ectopic pregnancy rate were compared between the two groups.Results:There was no significant difference in the mean age,body mass index,infertility time,proportion of primary infertility,and endometrial preparation protocol between the assisted hatching group and the control group(P>0.05);the clinical pregnancy rate of the assisted hatching group(76.62%)was higher than the control group(33.33%)(P<0.05);the implantation rate of the assisted hatching group(53.57%)was higher than the control group(22.62%)(P<0.05);the multiple pregnancy rate and ectopic in the two groups There was no significant difference in pregnancy rate(P>0.05).Conclusion:Laser assisted hatching can improve the clinical pregnancy rate and implantation rate of freeze-thaw cleavage embryo transfer,without increasing the risk of multiple pregnancy and ectopic pregnancy.
