摘要
目的:对比慢性化脓性中耳炎患者术前生理盐水外耳道冲洗和碳酸氢钠溶液冲洗的临床效果。方法:选取笔者所在医院2018年3月-2019年3月收治的慢性化脓性中耳炎患者64例,按随机数字表法分为碳酸氢钠组(n=32)与生理盐水组(n=32),两组均行耳内镜下单纯鼓膜修补术治疗,生理盐水组予以生理盐水外耳道冲洗,碳酸氢钠组予以碳酸氢钠溶液冲洗,对比两组耳内黏膜肿胀情况、耳内清洁度、手术时间、术中出血量及治疗前后pH值变化。结果:治疗后,碳酸氢钠组pH值高于生理盐水组,差异有统计学意义(P<0.05);碳酸氢钠组耳内黏膜肿胀情况优于生理盐水组,差异有统计学意义(P<0.05);碳酸氢钠组耳内清洁率与生理盐水组相比差异无统计学意义(P>0.05);碳酸氢钠组手术时间短于生理盐水组,术中出血量少于生理盐水组,差异均有统计学意义(P<0.05)。结论:碳酸氢钠溶液冲洗应用于慢性化脓性中耳炎患者术前准备中,会提高pH值,但可显著改善耳内黏膜肿胀情况,能缩短手术时间,减少术中出血量。
Objective:To compare the clinical effect of preoperative normal saline external ear canal irrigation and sodium bicarbonate solution irrigation in patients with chronic suppurative otitis media.Method:Sixty-four patients with chronic suppurative otitis media in our hospital from March 2018 to March 2019 were selected,and were randomly divided into the sodium bicarbonate group(n=32)and the saline group(n=32).All patients underwent endoscopic tympanic membrane repair.The saline group was treated with normal saline external ear canal.The sodium bicarbonate group was washed with sodium bicarbonate solution.The changes of intra-auricular mucosal swelling,intra-auricular cleanliness,operation time,intraoperative bleeding and pH before and after treatment were compared.Result:After treatment,the pH value of sodium bicarbonate group was higher than that of normal saline group,the difference was statistically significant(P<0.05).The swelling of mucous membrane in sodium bicarbonate group was better than that of normal saline group,the difference was statistically significant(P<0.05).There was no significant difference in intra-auricular cleanliness between sodium bicarbonate group and normal saline group(P>0.05).The operation time of sodium bicarbonate group was shorter than that of normal saline group,and the intraoperative blood loss was less than that of normal saline group,the differences were statistically significant(P<0.05).Conclusion:Sodium bicarbonate solution rinse in patients with chronic suppurative otitis media preoperative preparation,will increase the pH,but can significantly improve the swelling of the mucosa in the ear,can shorten the operation time and reduce the amount of intraoperative blood loss.
