摘要
目的:观察宫颈扩张球囊与控释地诺前列酮栓在初产妇促宫颈成熟及引产中的应用效果。方法:选取150例初产妇为研究对象,根据引产方式分为A组(宫颈扩张球囊引产)和B组(控释地诺前列酮栓引产)各75例,观察并记录两组宫颈成熟及分娩情况。结果:两组促宫颈成熟情况,引产前、后宫颈Bishop评分,剖宫产比例,新生儿窒息及产后出血发生比例比较差异无统计学意义。A组的缩宫素使用比例明显高于B组;总产程、临产时间较B组明显延长;羊水粪染、急产、子宫过度刺激发生比例较B组明显降低(P<0.05)。结论:在初产妇促宫颈成熟方面,宫颈扩张球囊与控释地诺前列酮栓效果相似,其中控释地诺前列酮栓作用快,能缩短产程,而宫颈扩张球囊作用温和,不良反应发生风险较低。
Objective:To observe the effect of cervical dilatation balloon and controlled release norprostone suppository on promoting the cervical ripening and inducing labor of primipara.Methods:One hundred and fifty primiparas were selected as the research subjects and were divided into group A(induced labor by cervical dilatation balloon)and B(induced labor by controlled release of dinoprostone suppository)with 75 cases each.Their cervical maturity and delivery were observed and recorded.Results:There were no significant differences between group A and B in cervical ripening and Bishop score before and after induction of labor and the incidence of cesarean section,neonatal asphyxia and postpartum hemorrhage(P>0.05).However,the rates of oxytocin use were significantly higher,the total delivery process and the time of parturition were significantly longer and the rates of amniotic fluid fecal contamination,emergency delivery and uterine hyperstimulation were significantly lower in group A than group B(P<0.05).Conclusion:The effect of cervical dilatation balloon is similar to that of controlled release of dinoprostone suppository in the aspect of promoting cervical ripening of primipara,in which the effect of controlled release of dinoprostone suppository is fast and it can shorten the delivery process,while the effect of cervical dilatation balloon is mild and the incidence of adverse reactions is low.
