摘要
目的探讨影响本院检验科凝血功能三项和D-二聚体(D-D)检验的主要分析前因素及处理措施。方法回顾分析本科室2019年1月至2019年6月住院患者的8651例凝血三项和D-D联合检测的标本状态及检验结果,标本检验采用BE血凝分析仪进行检测。结果本组标本中有质量问题的标本251例,其中采血量过少105例,抗凝剂与血液未充分混合导致标本有凝块的68例,从留置针中抽血肝素污染25例,输液过程中抽血导致血液稀释34例,未按要求空腹抽血重度脂血的标本19例。经电话通知临床后有78例标本重新送检后,检测的结果活化部分凝血酶原时间(aPTT、)凝血酶原时间(PT)均低于初次样本,纤维蛋白原(Fbg)均高于初次结果,差异均有统计学意义(P<0.01),D-D与原结果差异不大。结论标本采集量不准,采集方法不当,药物干扰,标本运输过程不合理等均可影响凝血三项和D-D结果的准确性。检验分析前标本的采集和运输正确合理是凝血功能检验结果准确的重要保证。
Objective To explore the main pre analysis factors and treatment measures affecting the three items of coagulation function and D-dimer(D-D)test in the laboratory of our hospital.Methods From January 2019 to June 2019,8651 inpatients in the undergraduate department were analyzed in terms of the sample status and test results of the combined detection of three items of coagulation and D-D The sample was tested by BE hemagglutination analyzer.Results There were 251 samples with quality problems in this group,including 105 samples with too little blood collection,68 samples with clots due to insufficient mixing of anticoagulant and blood,25 samples with heparin pollution from the retention needle,34 samples with hemodilution due to blood extraction during the infusion process,19 samples with severe fat blood not drawn as required.After the clinical call,78 samples were sent for re examination.The results showed that aPTT and PT were lower than those of the first samples,Fbg were higher than those of the first samples.The differences were statistically significant(P<0.01).D-D was not significantly different from the original results.Conclusion The accuracy of three coagulation items and D-D results can be affected by incorrect sample collection,improper collection method,drug interference and unreasonable sample transportation.It is an important guarantee for the accuracy of coagulation test results to collect and transport samples correctly and reasonably before test and analysis.
