Current Status and Clinical Significance of Prenatal Ultrasound Screening for Fetal Malformations in Parturients of Different Ages

摘要 目的探讨不同年龄阶段产前超声筛查胎儿畸形的现状及临床意义。方法回顾性分析2017年1月-2019年1月在我院进行门诊产前超声筛查孕妇4336例。根据孕妇年龄不同分为<20岁309例为A组、21~25岁1491例为B组、26~30岁1397例为C组、31~35岁715例为D组、>35岁424例为E组共5组,对比产前超声筛查结果与产前诊断中心确诊结果、引产尸检结果或产后检查结果。统计分析各组超声筛查胎儿畸形情况、畸形胎儿类型、畸形发生数目。结果各组超声筛查胎儿畸形中超声检出率E组最高且高于B、C、D组(P<0.05);各组超声筛查胎儿畸形中诊断符合率A组最高,D组最低,漏误诊率E组最高,但均无统计学差异(P>0.05);各组超声筛查胎儿畸形类型中E组胎儿畸形数目占比最高,且高于B、C、D组(P<0.05);所有畸形类型中先天性心脏畸形数最多且高于其他类型(P<0.05);各组超声筛查畸形中E组多发畸形检出率最高,但与其他组无统计学差异(P>0.05)。结论高龄孕妇以及年龄过较小孕妇易发生胎儿畸形,且高龄孕妇多发胎儿畸形几率较大,其中先天性心脏畸形和神经系统畸形较多见。 Objective To investigate the current status and clinical significance of prenatal ultrasound screening for fetal malformations in parturients of different ages.Methods A total of 4336 pregnant women underwent prenatal ultrasound screening in our hospital from January 2017 to January 2019 were analyzed,and women were divided into groups according to their ages,group A(<20 year-old,n=309),group B(21~25 year-old,n=309),group C(26~30 year-old,n=309),group D(31~35 year-old,n=309),and group E(>35 year-old,n=309).The results of prenatal ultrasound screening were compared with those of prenatal diagnosis center,autopsy of induced labor or postpartum examination.The ultrasound screening of fetal malformations,types of malformations,the number of malformations were recorded for statistical analysis.Results The detection rate of fetal malformations by ultrasound in group E was the highest,which was higher than that of the group B,C and D(P<0.05).The diagnostic coincidence rate was the highest in group A and the lowest in group D,andthe missed diagnosis rate was the highest in group E,with no significant difference with other groups(P>0.05).Among the types of fetal malformations screened by ultrasound,the number of fetal malformations in group E was the highest,which was significantly higher than that in group B,C and D(P<0.05).The number of congenital heart malformations was the highest among all malformations(P<0.05).The detection rate of multiple malformations was the highest in group E,showing no significant difference with other groups(P>0.05).Conclusion Fetal malformations have higher incidence rate among older and younger pregnant women,and older pregnant women are more likely to have multiple fetal malformations,among which congenital heart malformations and nervous system malformations are more common.
