摘要
目的研究独活寄生汤结合推拿对腰椎间盘突出患者的疗效及血栓素B2(TXB2)、肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)的变化。方法选取医院2016年12月-2018年8月收治的腰椎间盘突出患者108例,根据治疗方法不同分为对照组(采用推拿治疗)和观察组(采用独活寄生汤结合推拿治疗),分析两组患者治疗后的临床疗效。结果观察组总有效率94.44%(51/54),对照组为81.48%(44/54),观察组患者总有效率高于对照组(P<0.05)。两组治疗前视觉模拟(VAS)评分、日本骨科学会下腰痛(JOA)评分组间比较,差异不具有统计学意义(P>0.05)。观察组治疗后JOA评分高于对照组,VAS评分低于对照组(P<0.05)。两组治疗前TNF-α、IL-1β、白细胞介素-1(IL-1)、TXB2、降钙素基因相关肽(CGRP)、超氧化物歧化酶(SOD)水平组间比较,差异不具有统计学意义(P>0.05)。观察组治疗后SOD水平高于对照组,TNF-α、IL-1β、IL-1、TXB2、CGRP水平低于对照组(P<0.05)。两组治疗前后胫神经和腓总神经传导速度组间比较,差异不具有统计学意义(P>0.05)。结论独活寄生汤结合推拿治疗腰椎间盘突出疗效显著,可减轻患者疼痛程度,降低TXB2、TNF-α、IL-1β水平的表达,改善腰部功能。
Objective To study the curative effect of Duhuo Jisheng Decoction combined with Tuina on patients with lumbar disc herniation and the changes of thromboxane B2(TXB2),tumor necrosis factor-α(TNF-α)and interleukin-1β(IL-1β).Methods A total of 108 patients with lumbar disc herniation admitted in our hospital from December 2016 to August 2018 were divided into two groups:control group(treated with Tuina)and observation group(treated with Duhuo Jisheng Decoction combined with Tuina).We analyzed the clinical effect of two groups after treatment.Results The total effective rate of the observation group was 94.44%(51/54)and that of the control group was 81.48%(44/54).The total effective rate of observation group was higher than that of the control group(P<0.05).The VAS score and the JOA score of low back pain before treatment were compared between the two groups.The difference was not statistically significant(P>0.05).After treatment,the JOA score of the observation group was higher than that of the control group,and the VAS score was lower than that of the control group(P<0.05).TNF-α,IL-1β,interleukin-1(IL-1),TXB2,calcitonin gene related peptide(CGRP)and superoxide before treatment in the two groups were higher than those in the control group.There was no significant difference in the level of SOD between the two groups(P>0.05).After treatment,the levels of SOD,TNF-α,IL-1β,IL-1,TXB2 and CGRP in the observation group were higher than those in the control group(P<0.05).The difference was not statistically significant(P>0.05).Conclusion Duhuo Jisheng Decoction combined with Tuina has a significant effect on lumbar disc herniation.It can relieve the pain,reduce the expressions of TXB2,TNF-αand IL-1βand improve the lumbar function.
