摘要
目的调查无锡市水产养殖健康人群肠道中耐药细菌组成及耐药情况,为流行病学研究及临床合理用药提供参考依据。方法 2017年6月,采集无锡市两个渔村的水产养殖健康人群粪便样本,分别用含有头孢他啶(4μg/mL)和多粘菌素E(4μg/mL)的营养琼脂平板初步筛选能够生长的肠道耐药菌,鉴定细菌种类并用微量肉汤稀释法对分离得到的大肠埃希菌和克雷伯菌进行药物敏感性试验。结果共采集到符合条件的健康人群粪便标本175份,经过两种初筛平板的分离培养和细菌鉴定,得到245株大肠埃希菌和138株克雷伯菌。药敏试验结果显示,除替加环素外,大肠埃希菌和克雷伯菌对于其他12种抗菌药物均有不同程度的耐药。其中,大肠埃希菌耐药情况更为突出,对哌拉西林(41.7%)、四环素(48.7%)和复方新诺明(47.2%)的耐药率较高。克雷伯菌对四环素耐药率达31.2%,对其他药物耐药率均未超过30.0%。此外,大肠埃希菌的多重耐药现象也较为普遍,有超过80.0%的大肠埃希菌对3种及3种以上的药物产生耐药。结论无锡市水产养殖人员肠道中普遍存在耐药菌,其中大肠埃希菌的多重耐药现象较为严重,耐药传播的风险性较高。
Objective To investigate the composition and drug resistance of intestinal bacteria in healthy aquaculture population in Wuxi City,provide evidence for epidemiological research and clinical rational drug use. Methods In June 2017,fecal samples of healthy people in aquaculture were collected from two fishing villages in Wuxi City. The nutrient agar plate containing ceftazidime(4 μg/ml) and polymyxin E(4 μg/ml) were used to screen the intestinal resistant bacteria. The species of bacteria were identified,and the drug sensitivity of Escherichia coli and Klebsiella isolated were tested by broth dilution method. Results A total of 175 qualified fecal specimens of healthy people were collected in this survey. After the isolation,culture and bacterial identification of two kinds of screening plates,245 Escherichia coli strains and 138 Klebsiella strains were obtained. In addition to Tegacycline,Escherichia coli and Klebsiella have different degrees of resistance to 12 other antibiotic drugs. The drug resistance of Escherichia coli was more prominent,with higher drug resistance to Piperacillin(41.7%),Tetracycline(48.7%) and Sulfamethoxazole(47.2%).The drug resistance rate of Klebsiella to Tetracycline was up to 31.2%,and that to other drugs was over 30.0%. Moreover,multiple drug resistance of Escherichia coli was also common,with more than 80.0% of Escherichia coli resistant to three or more drugs.Conclusion Drug-resistant bacteria are common in the intestinal tract of aquaculture workers in Wuxi City,among which Escherichia coli has a serious phenomenon of multiple drug resistance and a high risk of drug resistance transmission.
