摘要
广义的伤寒是一切外感病的总称,温病亦属其中,六经辨证亦可运用于新型冠状病毒肺炎(corona virus disease 2019,COVID-19)各期的治疗中。预防期,以调节阴阳平衡,祛除湿邪为主,方用桂枝汤加茯苓、白术。素体痰湿较重者,方用防己黄芪汤加减。初期,病主在太阳或太阴。病在太阳,依证选用麻杏石甘汤加减、大青龙汤加减、桔梗汤等。病在太阴,方用理中汤加减。若太阳、太阴合病,方用桂枝去桂加茯苓白术汤加减。若太阳、少阴合病,方用麻黄细辛附子汤加减。中期,病主在少阳或阳明。病在少阳,方用小柴胡汤加减。病在阳明,依证选用白虎汤加减、小承气汤加减等。若少阳、阳明合病,方用大柴胡汤加减。若太阳、太阴合病,依证选用射干麻黄汤加减、葶苈大枣泻肺汤、小陷胸汤、麻黄连翘赤小豆汤加减等。若阳明、少阴合病,方用猪苓汤加减。极期,病主在阳明、少阴或厥阴。阳气暴脱证,可予参附注射液静脉滴注,气阴两虚证,可予生脉注射液静脉滴注。病在阳明,方用大承气汤加减。病在少阴,方用真武汤加减。病在厥阴,方用乌梅丸加减。恢复期,病主在阳明或太阴。病在阳明,方用竹叶石膏汤加减。病在太阴,方用参苓白术散加减。运用六经辨证可以了解疾病的发生发展过程,然疾病之传变方式多样,不可一概而论,辨证施治仍为第一要义。
In terms of treating corona virus disease 2019(COVID-19),in the prevention period,the balance of yin and yang should be adjusted and dampness should be eliminated.Cinnamon Twig Decoction plus Fuling(Poria)and Baizhu(Rhizoma Atractylodis Macrocephalae)is often used.For the patients with constant severe phlegm and dampness,Modified Stephania Root and Astragalus Decoction can be adopted.In the early period,the disease mainly lies in taiyang or taiyin meridians.For disease in taiyang meridians,Modified Ephedra and Apricot Kernel and Gypsum and Licorice Decoction,Modified Major Green Dragon Decoction,or Radix Platycodonis Decoction can be chosen.For disease in taiyin meridians,Modified Center-Regulating Decoction can be chosen.For disease in both taiyang and taiyin meridians,Modified Cinnamon Twig Decoction plus Fuling(Poria)and Baizhu(Rhizoma Atractylodis Macrocephalae)but minus Guizhi(Cinnamon Twig)can be adopted.For disease in both taiyang and shaoyin meridians,Modified Ephedra and Aconite and Asarum Decoction can be used.In the middle period,the disease mainly lies in shaoyang or yangming meridians.For disease in shaoyang meridians,Modified Minor Bupleurum Decoction can be used.For disease in yangming meridians,Modified White Tiger Decoction,or Modified Minor Purgative Decoction can be chosen.For disease in both shaoyang and yangming meridians,Modified Major Bupleurum Decoction can be used.For disease in both taiyang and taiyin meridians,Modified Belamcanda and Ephedra Decoction,or Descurainiae and Jujube Lung-Draining Decoction,or Minor Chest-Draining Decoction,or Modified Ephedra and Coptis and Semen Phaseoli Decoction can be chosen.For disease in both yangming and shaoyin meridians,Modified Polyporus Decoction can be used.In the extreme period,the disease mainly lies in yangming,shaoyin and jueyin meridians.For sudden collapse of yang qi,Ginseng and Aconite Injection can be used for intravenous drip.For qi-yin deficiency,Pulse-Engendering Injection can be used for intravenous drip.For disease in yangming meridians,Modified Major Purgative Decoction can be used.For disease in shaoyin meridians,Modified True Warrior Decoction can be used.For disease in jueyin,Modified Mume Pill can be used.In recovery period,the disease mainly lies in yanging or taiyin meridians.For disease in yangming meridians,Modified Lophatherum and Gypsum Decoction can be used.For disease in taiyin meridians,Modified Ginseng,Poria and White Atractylodes Powder can be used.The occurrence and development of diseases can be understood by syndrome differentiation of six meridians.However,there are various ways of disease transmission,which cannot be generalized.Therefore,syndrome differentiation and treatment is still the first important principle.
