摘要
目的探讨达芬奇机器人妇科手术中电器械相关并发症的原因及防治技巧。方法回顾性分析郑州大学第一附属医院妇科微创中心2014年10月至2019年6月1000例达芬奇机器人手术患者的临床资料,分析术中电器械相关并发症发生情况及原因。结果术中电器械相关并发症总发生率为2.5%(25/1000),包括血管损伤1.1%(11/1000),输尿管损伤1.1%(11/1000),膀胱损伤0.2%(2/1000),肠管损伤0.1%(1/1000)。结论电器械相关并发症与电器械使用不当、术者操作失误及系统故障有关。术中合理使用电器械预防机械损伤、直接热损伤及热效应,提高术者手术操作技巧和保证器械质量等是预防机器人妇科手术中电器械相关并发症的关键。
Objective To investigate the causes and prevention strategies of complications related to intraoperative electric instruments in da Vinci robot surgery of gynecological operation. Methods We retrospectively collected the clinical data of 1000 cases of da Vinci robot-assisted surgery from October 2014 to June 2019 in the First Affiliated Hospital of Zhengzhou University. Then we analyzed the occurrence and causes of intraoperative complications related to electric instruments. Results The overall incidence of complications was 2.5%(25/1000),including 1.1%(11/1000)for vascular injury,1.1%(11/1000)for ureteral injury,0.2%(2/1000)for bladder injury and 0.1%(1/1000)for bowel injury. Conclusion Electrical device-related complications are associated with the misuse of electric instruments,surgeons’ fault and system failure. In order to prevent intraoperative electric instruments-related complications in robotassisted surgery,it is essential to avoid any mechanical injury,direct thermal injury and thermal effects. Surgeons should improve surgical skills and ensure the quality of electric instruments used in the operation.
