摘要
目的了解2006-2018年桐乡市女性性工作者艾滋病病毒(HIV)流行状况和流行趋势,为制定相关策略提供参考。方法采用随机抽样法对2006-2018年桐乡市女性性工作者进行面对面问卷调查,每年调查人数不少于400人,共调查5 143人,了解人口学情况,采集静脉血样检测HIV、梅毒抗体,对梅毒感染采用Logistic回归分析影响因素。结果 2006-2018年桐乡女性性工作者艾滋病知晓率总体趋于平稳,2011年知晓率为94. 80%。2017年梅毒阳性率为6. 40%,其余年份为3%~5%,低档场所梅毒阳性率高于中、高档场所。Logistic回归分析结果显示,年龄、婚姻状况、文化程度、场所档次为女性性工作者梅毒抗体检测阳性的独立因素(P <0. 05)。同居或在婚为梅毒感染的保护因素,场所档次越低,检出梅毒阳性的风险增加。结论低档场所女性性工作者中梅毒阳性率偏高,应对该人群艾滋病、梅毒等经性接触传播疾病进行重点干预,推广安全套使用,加强与公安等执法部门合作。
Objective To understand the HIV prevalence and trend among female sex workers in Tongxiang from 2006 to2018,and provide reference for formulating relevant strategies. Methods A face-to-face questionnaire survey was carried out among female sex workers in Tongxiang from 2006 to 2018 by random sampling method. The number of people investigated was no less than 400 every year,a total of 5 143 people were investigated to understand the demographic situation. Venous blood samples were collected to detect HIV and syphilis antibodies. The risk factors of syphilis infection were analyzed by Logistic regression. Results From 2006 to 2018,the overall awareness rate of AIDS among the female sex workers in Tongxiang tended to be stable,and the awareness rate in 2011 was 94. 80%. In 2017,the positive rate of syphilis was6. 40%,and in other years,they were between 3% and 5%. The positive rate of syphilis in low grade places was higher than that in middle and high grade places. Logistic regression analysis showed that age,marital status,education level and place grade were independent factors of syphilis antibody test among the female sex workers( P < 0. 05). Cohabitation or marriage were the protective factor of syphilis infection. Lower level places increased the risk of syphilis detection. Conclusion The positive rate of syphilis in female sex workers in low level places is high. We should focus on the intervention of AIDS and syphilis through sexual contact,promote the use rate of condoms,and strengthen cooperation with law enforcement departments such as public security.
