摘要
目的探讨强迫症(OCD)与广泛性焦虑障碍(GAD)患者脑电超慢波信息变化及其与临床症状的相关性。方法选取OCD、GAD患者和健康成人(对照组)各50例,采用症状自评量表(SCL-90)评估临床症状,同时行脑电超慢波信息分析技术(EFG)检测。比较OCD组、GAD组、对照组SCL-90评分及EFG检测结果(脑电超慢波绝对功率值、脑电超慢波相对功率值、脑功能指数及全脑总功率),并分析两者的相关性。结果3组对象SCL-90各因子评分及总分比较,差异均有统计学意义(均P<0.05);OCD组、GAD组SCL-90各因子评分及总分均高于对照组(均P<0.05);与GAD组比较,OCD组强迫因子评分较高(P<0.05),躯体化、抑郁、焦虑、恐怖因子评分均较低(均P<0.05)。与对照组比较,OCD组2mHz(谷氨酸)、11mHz(多巴胺)绝对功率值及2mHz(谷氨酸)相对功率值和GAD组4mHz(5-羟色胺)、7mHz(去甲肾上腺素)绝对功率值均较高(均P<0.05),OCD组4mHz(5-羟色胺)绝对功率值及4mHz(5-羟色胺)、6mHz(兴奋因子6)、7mHz(去甲肾上腺素)相对功率值和GAD组1mHz(-氨基丁酸)绝对功率值、1mHz(-氨基丁酸)相对功率值均较低(均P<0.05);与GAD组比较,OCD组1mHz(-氨基丁酸)、2mHz(谷氨酸)绝对功率值及相对功率值均较高(均P<0.05),4mHz(5-羟色胺)绝对功率值及相对功率值均较低(均P<0.05),7mHz(去甲肾上腺素)绝对功率值亦较低(P<0.05);脑功能指数及全脑总功率比较,仅见GAD组运动指数低于对照组(P<0.05)。3组对象EFG检测结果与SCL-90总分作相关性分析,仅见GAD组4mHz(5-羟色胺)相对功率值与SCL-90总分呈负相关(P<0.05)。结论OCD、GAD患者脑电超慢波信息变化存在差异,GAD患者4mHz(5-羟色胺)相对功率值与临床症状相关。EFG检测简单易行、无创伤性,对OCD、GAD的临床诊断与鉴别诊断具有一定的指导作用。
Objective To investigate the correlation of electroencephalograph(EEG)superslow wave information with clinical symptoms in patients with obsessive-compulsive disorder(OCD)and generalized anxiety disorder(GAD).Methods Fifty patients with OCD,50 patients with GAD and 50 healthy adults(control group)were enrolled in the study.The symptom checklist(SCL-90)was used to evaluate the clinical symptoms,and the EEG superslow wave information analysis(encephalofluctuography,EFG)was performed.The examination results(absolute power value,relative power value,brain function index and total brain power)were compared among OCD,GAD and control groups,and the correlation between SCL-90 scores and EFG results was analyzed.Results The scores of SCL-90 factors and the total scores of the three groups were statistically significant(P<0.05).The scores of SCL-90 factors and the total score of the OCD group and GAD group were higher than those of the control group(P<0.05).Compared with the GAD group,the OCD group had higher compulsive factor scores(P<0.05),while the somatization,depression,anxiety and terror factor scores were lower(P<0.05).Compared with the control group,the absolute power values of 2mHz(glutamic acid),11mHz(dopamine)and relative power values of 2mHz(glutamic acid)in the OCD group and the absolute power values of 4mHz(5-hydroxytryptamine)and 7mHz(norepinephrine)in the GAD group were higher(P<0.05).And,the absolute power of 4mHz(5-hydroxytryptamine)and relative power of 4mHz(5-hydroxytryptamine),6mHz(excitation factor 6),7mHz(norepinephrine)in the OCD group and the absolute power of 1mHz(-aminobutyric acid)and the relative power values of 1mHz(-aminobutyric acid)in the GAD group were lower(P<0.05).Compared with the GAD group,the absolute and relative power values of 1mHz(-aminobutyric acid)and 2mHz(glutamic acid)were higher in the OCD group(P<0.05),and the absolute and relative power values of 4mHz(5-hydroxytryptamine)were lower(P<0.05),and the absolute power value of 7mHz(norepinephrine)was also lower(P<0.05).In brain function index and total brain power,the exercise index of GAD group was lower than that of control group(P<0.05).The correlation analysis showed that only the relative power value of 4mHz(5-hydroxytryptamine)in the GAD group was negatively correlated with the total score of SCL-90(P<0.05).Conclusion There are differences in the EEG superslow wave information between patients with OCD and GAD.The relative power of 4mHz(5-hydroxytryptamine)in patients with GAD is related to clinical symptoms.EFG detection is simple and non-invasive,and has a certain value in clinical diagnosis and differential diagnosis of OCD and GAD.
