摘要
目的探讨颈部"L"形切口常规手术与颈部低位延长领式切口手术治疗甲状腺癌患者的临床效果。方法选取本院2016年1月至2018年12月收治的76例甲状腺癌患者,随机分为对照组(n=38)及研究组(n=38),对照组予以传统颈部"L"形切口常规手术治疗,研究组予以颈部低位延长领式切口手术治疗,对比两组临床手术指标、术后并发症发生情况、术后满意度及患者术前、术后生活质量评分。结果研究组的临床手术指标、术后满意度均优于对照组(P<0.05),两组评分显示患者术前SF-36评分未见明显差异(P>0.05),术后对照组患者的评分明显低于研究组(P<0.05);研究组术后继发出血发生率明显低于对照组(P<0.05),喉返神经受损、颈淋巴瘘、甲状腺功能减退、皮瓣坏死的发生率上两组比较无明显差异(P>0.05)。结论颈部低位延长领式切口手术治疗甲状腺癌切口小、并发症少,患者术后满意度高,能有效提高患者生活质量,值得在临床上推广。
Objective To investigate the clinical effect of conventional surgery with jugular "L" incision and surgery with low jugular extended collar-shaped incision in treatment of thyroid cancer. Method 76 patients with thyroid cancer admitted to our hospital from January 2016 to December 2018 were selected and randomly divided into the control group(n=38) and the study group(n=38). The control group was treated with conventional surgery with jugular "L" incision in the neck. The study group was treated with surgery with low jugular extended collar-shaped incision. The clinical surgical indexes, postoperative complications, postoperative satisfaction, and preoperative and postoperative quality of life scores of patients were compared between the two groups. Results The clinical index and postoperative satisfaction of the study group were better than those of the control group(P<0.05). The scores of the two groups showed no significant difference in the preoperative SF-36 score(P>0.05). After operation, the scores of the control group were significantly lower than those of the study group(P<0.05). The incidence of postoperative secondary hemorrhage in the study group was significantly lower than that of the control group(P<0.05). There was no significant difference in the incidence of recurrent laryngeal nerve injury, jugular lymphatic fistula, hypothyroidism, and skin flap necrosis(P>0.05). Conclusion The surgery with low jugular extended collar-shaped incision in treatment of thyroid cancer has small incision, less complications, high postoperative satisfaction, and can effectively improve the quality of life of patients. It is worthy of clinical promotion.
