摘要
目的分析妊娠期糖尿病妊娠合并甲状腺功能减退对心功能及妊娠结局的影响。方法回顾性分析2018年1月—2019年1月该院产科接受诊治的92例妊娠期糖尿病患者资料进行分析,根据其是否合并存在甲状腺功能减退症状,纳入甲减组与常规组(n=46)。为入选患者实施甲状腺功能相关指标的检查,且对其心功能状态予以检测。比较两组患者的妊娠结局及并发症发生率。结果甲减组患者TSH值、FT3值及FT4均值均明显下降,治疗后甲减组MW的E峰均数为(74.41±12.22)cm/s,常规组E峰均数为(89.01±20.17)cm/s,差异有统计学意义(t=8.06,P=0.000)。甲减组患者的不良妊娠结局发生率为28.26%,常规组不良妊娠结局发生率为13.04%,两组产妇相比,差异有统计学意义(χ2=6.03,P=0.035)。结论妊娠期糖尿病患者合并存在甲状腺功能减退症状,会直接影响患者的心功能状态,增加患者不良妊娠结局发生率,需要早期予以临床重视,实施对症治疗,避免患者不良问题的发生。
Objective To analyze the effects of gestational diabetes mellitus combined with hypothyroidism on cardiac function and pregnancy outcomes.Methods Retrospectively analyzed the data of 92 gestational diabetes patients admitted to the hospital from January 2018 to January 2019.Based on whether they were combined with hypothyroidism,they were included in the hypothyroidism group and the conventional group(n=46).The selected patients were examined for thyroid function-related indicators,and their cardiac function status was tested.The pregnancy outcomes and complications were compared between the two groups.Results The TSH value,FT3 value and FT4 mean value of patients with hypothyroidism group decreased significantly.The mean E-peak value of MW in hypothyroidism group after treatment was(74.41±12.22)cm/s,and the average E-peak value of conventional group was(89.01±20.17)cm/s,the difference between the two groups is obvious,the difference was statistically significant(t=8.06,P=0.000).The incidence of adverse pregnancy outcomes in patients with hypothyroidism was 28.26%,and the incidence of adverse pregnancy outcomes in routine groups was 13.04%.There was a significant difference between the two groups(χ2=6.03,P=0.035).Conclusion The presence of hypothyroidism in patients with gestational diabetes mellitus will directly affect the patient's cardiac function and increase the incidence of adverse pregnancy outcomes.Early clinical attention is needed to implement symptomatic treatment to prevent adverse problems in patients.
