摘要
目的了解2018年易县8~10岁儿童甲状腺肿大情况,为碘营养干预提供依据。方法全县按东西南北中5个方位随机抽取1个乡镇,每个乡镇抽取2所小学,每所小学抽取8~10岁非寄宿学生不低于40人,对所抽取小学校的学生进行甲状腺肿触诊检查。结果2018年共调查8~10岁儿童1274人,甲状腺肿大43例,甲状腺肿大率3.38%。1度肿大36例,甲状腺肿大占有比83.72%;2度肿大7例,甲状腺肿大占有比16.28%。不同年龄(χ^2=0.1544)、性别(χ^2=1.4582)、区域(χ^2=0.6936)、民族(χ^2=0.0043)儿童甲状腺肿大率比较,差异均无统计学意义(P>0.05)。不同年龄(H=0.0159)、性别(Z=1.2182)、区域(H=0.0679)、民族(H=0.0005)儿童甲状腺肿大程度分布比较,差异均无统计学意义(P>0.05)。结论易县8~10岁儿童甲状腺肿大率在国家要求的范围之内,达到了碘缺乏病消除标准。今后应继续开展甲状腺肿大调查和碘营养的干预。
Objective To get an insight into the situation of goiter in children aged 8~10 years in Yi County in 2018,and to provide evidence for iodine nutrition intervention.Methods A total of 5 towns were randomly selected in the range of the county according to azimuth(east,west,south,north and center),respectively.Two primary schools were randomly chosen in every town of the five.And more than 40 non-boarding students aged 8~10 years were selected from each primary,school,and a thyroid palpation examination was performed on the children from primary schools.Results In 2018,a total of 1274 children aged 8~10 years were investigated,including 43 cases of goiter,with the goiter rate of 3.38%.The proportion of goiter was 83.72%in 36 cases with 1 degree enlargement,and the proportion of goiter was 16.28%in 7 cases with 2 degree enlargement.There were no significant differences in the prevalence of goiter in children of different ages(χ^2=0.1544),genders(χ^2=1.4582),regions(χ^2=0.6936),and ethnicities(χ^2=0.0043)(P>0.05).There was no significant difference in the distribution of goiter between children of different ages(H=0.0159),genders(Z=1.2182),regions(H=0.0679)and ethnicities(H=0.0005)(P>0.05).Conclusion The rate of goiter of 8~10 years old children in Yi County is within the range of national requirements,and has reached the elimination standard of iodine deficiency disorders.The investigation of goiter and the intervention of iodine nutrition should be continued in the future.
