高频超声在鉴别诊断婴儿胆道闭锁和婴儿肝炎综合征的应用价值 被引量:2

The value of high frequency ultrasound in the diagnosis of biliary atresia and infantile hepatitis syndrome in children

摘要 目的:应用高频超声对梗阻性黄疸婴儿进行检查,评价其在鉴别诊断早期小儿胆道闭锁(Bili Aryatresia,BA)与婴儿肝炎综合征(Infantile Hepatitis Syndrome,IHS)中的应用价值。方法:选取本院2017年9月-2019年6月收治的病理证实的62例胆道闭锁患儿和63例婴儿肝炎综合征患儿作为研究对象,均空腹4 h后进行腹部高频超声检查,记录了每例患儿的肝脏、胆囊、肝管、肝右动脉及"TC(Triangular Cord)征"的情况等,并初步判断是否为胆道闭锁,回顾性分析其高频超声图像结果。结果:以术后病理学检查结果为金标准,"TC征"阳性、胆囊大小形态及收缩功能异常、肝内外胆管未探及、肝右动脉增宽,对诊断BA都具有很高的价值,敏感度分别为61%、93%、90%、84%,特异度分别为100%、88%、95%、90%,准确度分别为81%、86%、93%、87%。肝门区孤立小囊出现率不高,对BA诊断的特异度很高。五种声像图联合判断敏感度97%。结论:高频超声能清晰显示诊断BA的特征性声像图,显著的提高了早期胆道闭锁诊断的准确性。 Objective:To analysis the diagnostic value of high-frequency ultrasound for BA and IHS with obstructive jaundice.Methods:62 cases of children with BA and 63 cases of IHS were chosen as objects of study,According to high-frequency ultrasound,the liver,gallbladder,hepatic duct,right hepatic artery and TC in children were recorded,to given preliminary judgment for BA,and highfrequency ultrasound image results were analyzed retrospectively.Results:The postoperative examination results as the gold standard,TC positive,gallbladder size and systolic function abnorma,not out of bile duct of liver inside and outside and widened,right hepatic artery,has a very high value for diagnosis of BA.Respectively,the sensitivity of them were 61%,95%,90%,84%,the specificity were 100%,88%,95%,90%,the accuracy were 81%,86%,93%,87%.The incidence of of isolated vesicles in hilar area is not high,the diagnosis accuracy of BA with the above signs is very high.The specificity of five ultrasonographic joint judgement was 97%.Conclusions:High frequency ultrasound can clearly show the characteristic appearance of BA,and significantly improve the accuracy of early biliary atresia diagnosis in children.
