摘要 目的观察补硒治疗对自身免疫性甲状腺炎免疫功能的影响。方法选择2018年3月-2019年3月我院收治的自身免疫性甲状腺炎患者105例。根据有无使用硒剂分为观察组53例和对照组52例。对照组给予常规治疗,观察组在对照组基础上给予硒酵母;2组均治疗6个月。比较2组治疗前后血清游离三碘甲状腺原氨酸(FT3)、血清游离甲状腺素(FT4)、促甲状腺激素(TSH)及甲状腺过氧化物酶抗体(TPOAb)水平;检测2组CD4+/CD8+、CD4+及CD8+细胞百分比。记录2组药物不良反应。结果 2组治疗前后FT3、FT4、TSH比较差异均无统计学意义(P>0.05)。观察组治疗后TPOAb低于治疗前和对照组(P<0.05)。2组治疗前后CD8+比较差异无统计学意义(P>0.05)。观察组治疗后CD4+/CD8+、CD4+均高于治疗前及对照组(P<0.05)。2组不良反应发生率比较差异无统计学意义(P>0.05)。结论硒治疗能够调节自身免疫性甲状腺炎患者免疫功能,降低甲状腺自身抗体水平。 Objective To observe the effect of selenium supplementation therapy on the immune function of patients with autoimmune thyroiditis(AIT). Methods From March 2018 to March 2019, 105 patients with AIT who were treated in our hospital were selected. According to the use of selenium agent, they were divided into the observation group(n=53) and the control group(n=52). The control group was given conventional treatment, while the observation group was given selenium yeast on the basis of the control group. Both groups were treated for 6 months. The levels of serum free triiodothyronine(FT3), serum free thyroxine(FT4), thyrotropin(TSH) and thyroid peroxidase antibody(TPOAb) were measured before and after treatment, and the percentage of CD4+/CD8+, CD4+ and CD8+ cells were measured. The adverse drug reactions of the two groups were compared. Results There was no significant difference in FT3, FT4 and TSH between the two groups before and after treatment(P>0.05). The TPOAb in the observation group was lower than that before treatment and that in the control group(P<0.05). There was no significant difference in CD8+ between the two groups before and after treatment(P<0.05). After treatment, the CD4+/CD8+ and CD4+ in the observation group were higher than those before treatment and those in the control group(P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05). Conclusion Selenium supplementation therapy can regulate the immune function of patients with AIT and reduce the level of thyroid autoantibody.
