摘要
目的研究超声心动图评价扩张型心肌病(DCM)患者左房容积和功能的临床价值。方法选取2016年12月至2018年12月我院DCM患者82例及同期健康体检者60例,采用常规超声测量左室舒张末期内径(LVEDD)、收缩末期内径(LVESD)、射血分数(LVEF)、测量E、A峰值速度并计算E/A,然后测量A峰速度-时间积分(AVTI)和二尖瓣口血流总积分(TVTI)并计算左房充盈分数(LAFF)和房缩A波速度(PVA),测量二尖瓣口面积(MVA)并计算左房射血分数(LAEF),计算左房容积(LAV)、左房容积指数(LAI)及左房扩张指数(LAEI)。比较两组各项指标差异并分析左房各参数与E/A相关性。结果DCM组LVEDD、LVESD、LVEF、LAVmax、LAVmin、LAVImax、LAFF和LAEI均高于对照组,E/A和LAEF均低于对照组,差异均有统计学意义(P<0.05);Pearson积差相关性分析显示,LAVmax、LAVmin和LAFF与E/A呈明显负相关性(P<0.05),LAEF、LAVImax及LAEI与E/A呈明显正相关性(P<0.05)。结论超声心动图可准确测量DCM患者左房容积和功能,为准确评估患者心功能状态提供有效参考依据,对临床诊断、病情监测和预后评估均具有重要意义。
Objective To study the clinical value of echocardiography in the evaluation of left atrial volume and function in patients with dilated cardiomyopathy(DCM).Methods 82 patients with DCM and 60 healthy examiners in our hospital from December 2016 to December 2018 were selected.The routine ultrasound was used to measure the left ventricular end-diastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD)and left ventricular ejection fraction(LVEF),and E and A peak velocities were measured and E/A was calculated,and the A peak velocity-time integral(AVTI)and total mitral valve flow integral(TVTI)were assessed and left atrial filling fraction(LAFF)and atrial systolic A-wave velocity(PVA)were calculated,and the mitral valve orifice area(MVA)was measured and the left atrial ejection fraction(LAEF)was calculated,and the left atrial volume(LAV),left atrial volume index(LAI)and left atrial expansion index(LAEI)were calculated.The differences of indexes were compared between the two groups,and the correlation between left atrial parameters and E/A was analyzed.Results The LVEDD,LVESD,LVEF,LAVmax,LAVmin,LAVImax,LAFF and LAEI in DCM group were higher than those in control group while the E/A and LAEF were lower than those in control group(P <0.05).Pearson product moment correlation analysis showed that LAVmax,LAVmin,LAVImax and LAFF were significantly negatively correlated with E/A(P<0.05),and LAEF and LAEI were significantly positively correlated with E/A(P<0.05).Conclusion Echocardiography can accurately measure the left atrial volume and function of DCM patients,and provide an effective reference for accurate assessment of cardiac function status.It is of great significance for clinical diagnosis,disease monitoring and prognosis evaluation.
