摘要
目的研究拇、手指旋转撕脱性完全离断再植Ⅰ期行肌腱神经转位修复效果及安全性。方法选取2018年3月~2019年3月在我院诊治的46例拇、手指旋转撕脱性完全离断患者为研究对象,采用再植Ⅰ期行肌腱神经转位修复治疗,观察临床治疗总优良率、治疗前后手功能(拇指运动、感觉、工作能力评分)情况、背伸肌力和屈曲肌力改善情况、外形满意度以及并发症(血栓、皮肤坏死)情况。结果46例全部成活,总优良率为93.47%;治疗后拇指运动、感觉、工作能力评分均高于治疗前,背伸肌力、屈曲肌力改善优于治疗前(P<0.05);术后随访患者满意度为95.65%,并发症发生率为6.52%。结论拇、手指旋转撕脱性完全离断再植Ⅰ期行肌腱神经转位修复成活率高,拇、手指功能恢复良好,且外观满意度高,疗效确切。
Objective To study the repair effect and safety of the tendon nerve transposition in stageⅠof complete avulsion and replantation of the thumb and fingers.Methods 46 patients with complete rotatory avulsion of the thumb and fingers who were treated in our hospital from March 2018 to March 2019 were selected as the research subjects,and the tendon nerve transposition repair was performed in stageⅠreplantation,observing the overall excellent and good rate of clinical treatment,hand function(thumb movement,sensation,work ability score)before and after treatment,improvement of extensor and flexor muscle strength,appearance satisfaction,and complications(thrombosis,skin necrosis).Results All of the 46 patients were survived,with a total excellent and good rate of 93.47%.After treatment,the thumb exercise,sensation,and work ability scores were higher than before treatment,the improvement of dorsiflexor and flexor muscle strength was better than that before treatment(P<0.05);the postoperative follow-up patient satisfaction was 95.65%,and the complication rate was 6.52%.Conclusion The thumb and fingers are completely avulsed and replanted in stageⅠ,Tendon nerve transposition repair is performed for a high survival rate,the function of thumb and fingers is well restored.
