摘要
目的髌骨骨折发生率呈逐年上升趋势,恢复髌骨解剖位置和伸屈功能是目前治疗髌骨骨折的关键,本研究探讨髌骨骨折患者采取空心钉张力带钢丝联合内固定治疗对关节功能及并发症影响。方法选取2016-01-10-2018-01-10南阳市第二人民医院收治的髌骨骨折患者作为研究对象,在患者知情同意前提下,按照性别、年龄和致伤原因组间均衡的原则,选择对照组与观察组,各42例,对照组采取克氏针张力带钢丝内固定,观察组采取空心钉张力带钢丝联合内固定,比较两组治疗手术时间、术中失血量、骨折愈合时间、关节功能及并发症发生率。结果观察组手术时间为(55.63±7.62)min,长于对照组的(50.13±5.28)min,t=3.845,P<0.05;术中失血量为(85.43±4.52)mL,多于对照组的(80.24±3.61)mL,t=5.815,P<0.05;骨折愈合时间为(5.09±0.58)个月,短于对照组的(6.96±0.67)个月,t=13.676,P<0.05。治疗后,观察组膝关节评分量表(knee score scale,HSS)中疼痛评分为(24.38±2.43)分,高于对照组的(19.87±2.31)分,t=8.718,P<0.05;功能评分为(17.49±3.05)分,高于对照组的(13.65±2.51)分,t=6.300,P<0.05;活动度评分为(14.74±2.69)分,高于对照组的(11.52±2.63)分,t=5.547,P<0.05;肌力评分为(7.41±1.26)分,高于对照组的(5.62±1.34)分,t=6.307,P<0.05;屈曲评分为(7.58±1.47)分,高于对照组的(5.79±1.32)分,t=5.872,P<0.05;稳定性评分(7.69±1.20)分,高于对照组的(5.63±1.28)分,t=7.609,P<0.05。观察组并发症发生率为4.76%,低于对照组的21.43%,χ^2=5.126,P<0.05。结论髌骨骨折患者采取空心钉张力带钢丝联合内固定治疗可促进关节功能改善,促进骨折愈合,降低并发症发生率。
OBJECTIVE The incidence of patella fractures is increasing year by year.The restoration of patella anatomical position and extension and flexion functions is the key to the treatment of patella fractures.This study explores the effects of hollow nail tension band steel wire and internal fixation treatment on patella fractures and joint function.METHODS Patients with patella fractures admitted to Nanyang Second People’s Hospital from January 10,2016 to January 10,2018 were selected as the research subjects.Based on the patient’s informed consent,according to the principle of balance between genders,ages and causes of injury,The control group and the observation group were selected,42 cases in each group.The control group was internally fixed with Kirschner wire tension band steel wire,and the observation group was combined with hollow nail tension band steel wire internal fixation.The surgical time,blood loss,fracture healing time,joint function and complication rate were compared between the two groups.RESULTS The operation time in the observation group was(55.63±7.62)min,which was longer than the control group(50.13±5.28)min,t=3.845,P<0.05;the blood loss during the operation was(85.43±4.52)ml,which was more than the control group(80.24±3.61)ml,t=5.815,P<0.05;fracture healing time was(5.09±0.58)months,shorter than the control group(6.96±0.67)months,t=13.676,P<0.05.After treatment,the pain score in the observation group(knee score scale,HSS)was(24.38±2.43)points,which was higher than the control group(19.87±2.31)points,t=8.718,P<0.05;the functional score was(17.49±3.05)points,which was high than the control group(13.65±2.51)points,t=6.300,P<0.05;activity score was(14.74±2.69)points,which was higher than the control group(11.52±2.63)points,t=5.547,P<0.05;The muscle strength score was(7.41±1.26)points,which was higher than the control group(5.62±1.34)points,t=6.307,P<0.05;the flexion score was(7.58±1.47)points,which was higher than the control group(5.79±1.32)points,t=5.872,P<0.05;stability score was(7.69±1.20)points,which was higher than the control group(5.63±1.28)points,t=7.609,P<0.05.The incidence of complications in the observation group was 4.76%,which was lower than the control group(21.43%),χ^2=5.126,P<0.05.CONCLUSION In patients with patella fractures,the use of hollow nail tension band steel wire combined with internal fixation can improve joint function,promote fracture healing,and reduce the incidence of complications.
