摘要
目的耐多药肺结核(multidrugresistant tuberculosis,MDR-TB)是经呼吸道传播的一种疾病,在临床上该疾病治疗失败率与致死率较高。本研究旨在比较含莫西沙星与含左氧氟沙星方案治疗耐多药肺结核的疗效,为临床应用提供参考。方法选择2018-03-21-2019-03-18长葛市人民医院收治的100例MDR-TB患者为研究对象,按照组间性别、年龄和病程匹配的原则,分为观察组与对照组,各50例。观察组采用含莫西沙星方案治疗,对照组采用含左氧氟沙星方案治疗,观察并记录患者病灶吸收情况和痰菌阴转率。结果观察组治疗总有效率为72.0%,高于对照组的46.0%,差异有统计学意义,χ^2=6.986,P=0.008。治疗3个月观察组痰菌阴转率为42.0%,高于对照组的22.0%,差异有统计学意义,χ^2=4.056,P=0.044;治疗6个月观察组痰菌阴转率为56.0%,高于对照组的32.0%,差异有统计学意义,χ^2=3.954,P=0.047;治疗12个月观察组痰菌阴转率为72.0%,高于对照组的50.0%,差异有统计学意义,χ^2=5.086,P=0.024。结论含莫西沙星方案治疗MDR-TB比含左氧氟沙星方案效果更好,痰菌阴转率更高,可优先参考使用。
OBJECTIVE Multidrug-resistant pulmonary tuberculosis(MDR-TB)is a disease transmitted through the respiratory tract,and its treatment failure rate and mortality rate are high in clinic.The purpose of this study was to compare the efficacy of regimens containing moxifloxacin and levofloxacin in the treatment of MDR-TB.METHODS A total of 100 MDR-TB patients treated in Changge People’s Hospital from March 21,2018 to March 18,2019 were selected as the study subjects.They were divided into observation group and control group according to the principle of matching gender,age and course of disease,with 50 cases in each group.The observation group was treated with moxifloxacin-containing regimen,while the control group was treated with levofloxacin-containing regimen.The absorption of lesions and sputum negative conversion rate were observed and recorded.RESULTS The total effective rate of the observation group was 72.0%,higher than 46.0%of the control group,and the difference was statistically significant,χ^2=6.986,P=0.008.The sputum negative conversion rate of the observation group was 42.0%after 3 months of treatment,which was higher than that of the control group(22.0%),and the difference was statistically significant,χ^2=4.056,P=0.044.After6 months of treatment,the sputum negative conversion rate of the observation group was 56.0%,which was higher than that of the control group(32.0%),and the difference was statistically significant,χ^2=3.954,P=0.047.After 12 months of treatment,the sputum negative conversion rate of the observation group was 72.0%,which was higher than that of control group(50.0%)and the difference was significant,χ^2=5.086,P=0.024.CONCLUSION Moxifloxacin-containing regimen is more effective than levofloxacin-containing regimen in the treatment of MDR-TB,the sputum negative conversion rate is higher,and can be used as a priority.
