摘要 大血管病变是糖尿病患者致残致死的主要原因之一,中医药防治优势突出。但由于历史认识的局限性,中医缺乏对糖尿病大血管病变系统深入和全面的理论阐释,严重制约了防治策略的优化。“脉积学说”是基于“癥瘕积聚”理论而提出,其核心病机是气血津液代谢失常,主要病理产物为痰浊、瘀血。治疗上本学说强调分类、分期、分证动态辨治,活血化痰贯穿于疾病治疗始终,并重视以鳖甲煎丸为代表丸剂的临床应用。本文对“脉积”学说阐述将有助于深化中医药对糖尿病大血管病变的认识,并为临床应用提供新的思路与方法。 Macrovascular disease is one of the main causes of disability and death in diabetic patients.TCM has prominent advantages in preventing and treating this disease,but due to the limitations of historical understanding,there is a lack of in-depth and comprehensive theoretical explanation of diabetic macrovascular diseases,which seriously restricts the optimization of prevention and treatment strategies."Mai-ji"theory is based on the theory of"Zhengia and Ji'Ju".The core pathogenesis is the disorder of Qi,blood and body fluid metabolism,and the main pathological products are phlegm turbidity and blood stasis.In terms of treatment,this theory emphasizes classification,staging and dynamic treatment of different syndromes.The concept of promoting blood circulation and resolving phlegm has been throughout the treatment of diseases,and it also attaches importance to the clinical application of pills represented by Beijiajian pill.This paper expounds the theory of"Mai-ji",which will help deepen the understanding of diabetic macrovascular diseases in traditional Chinese medicine,and provide new ideas and methods for clinical application.
