丙戊酸与托吡酯影响学龄期癫痫患儿认知功能、骨代谢以及血小板的随机对照研究 被引量:2

摘要 目的探讨丙戊酸与托吡酯影响学龄期癫痫患儿认知功能、骨代谢以及血小板的随机对照研究。方法选取2018年1月~2019年1月收治的80例学龄期癫痫患儿作为研究对象,将所有患儿随机等分为研究组与参照组,参照组共40例给予丙戊酸治疗,研究组共40例给予托吡酯治疗,研究两种药物对两组患儿血小板、骨代谢及认知功能的影响。结果治疗前两组患儿认知功能比较无显著差异(P>0.05),治疗后研究组患儿的认知功能相较于治疗前降低,差异具有统计学意义(P<0.05);两组患儿治疗前其骨代谢相关指标无显著差异(P>0.05),治疗后参照组骨代谢相关指标下降,与研究组间差异具有统计学意义(P<0.05);两组患儿在治疗6个月时其TXB2、血小板聚集率及血小板计数均显著下降,且研究组下降幅度大于参照组,组间差异有统计学意义(P<0.05),治疗前后两组患儿6-Keto-PGF1α无显著差异(P>0.05)。结论托吡酯治疗学龄期癫痫患儿疗效明显优于丙戊酸,显著促进其生长发育,对患儿骨代谢功能产生较大影响。托吡酯对患儿认知功能方面存在不足,安全性低于丙戊酸,在治疗过程中应定期监测患儿脑电图,托吡酯还对患儿血小板计数与功能产生影响,利于维护癫痫患儿身体稳定性。 Objective To investigate the effects of valproic acid and topiramate on cognitive function,bone metabolism and platelet in children with school-age epilepsy.Methods From January 2018 to January 2019,a total of 80 cases of school-age children with epilepsy as the research objects to carry on the analysis,all children were randomly divided into research group and control group.The control group a total of 40 cases was given valproic acid,the team of research group a total of 40 cases was with topiramate treatment,then the platelets,bone metabolism and the influence of cognitive function of the two groups were directly comparable.Results There was no significant difference in cognitive function between the two groups before treatment(P>0.05),but the cognitive function of the children in the research group decreased after treatment compared with that before treatment,and the difference was significant(P<0.05).There was no significant difference between the two groups of children on the bone metabolism before treatment(P>0.05).After treatment,the relevant indexes of bone metabolism decreased in the control group,with statistically significant difference between the two groups(P<0.05).At 6 months after treatment,TXB2,platelet aggregation rate and platelet count of the two groups of children significantly decreased,and the decline rate of the research group was greater than that of the control group,with significant difference between the groups(P<0.05),while there was no significant difference in 6-keto-pgf1α between the two groups before and after treatment(P>0.05).Conclusion The curative effect of topiramate treatment on school-age children with epilepsy is superior to valproic acid treatment,and have a greater impact on bone metabolism in children with significantly.The effect of topiramate on cognitive function in children with epilepsy,However,was with some disadvantages,the security is lower,and the treatment should be monitored regularly with EEG in the process.The treatment of topiramate also affect children with platelet count and function,to maintain epileptic children with physical stability.
