摘要
目的分析非小细胞肺癌(NSCLC)患者外周血CD4^+T淋巴细胞亚群与自然杀伤细胞(NK细胞)表达及其临床意义。方法本院收治的94例NSCLC患者(观察组)及同期健康体检者89例(健康组)。比较不同人群,不同病理类型、不同临床分期,有、无淋巴结转移NSCLC患者外周血CD4^+T淋巴细胞亚群、自然杀伤细胞情况。结果NSCLC组NK细胞、Th1、Th1/Th2、Th17、Th17/Treg表达低于健康组,Th2、Treg表达高于健康组(P<0.05);鳞癌与腺癌患者NK细胞、Th1、Th2、Th1/Th2表达比较,差异无统计学意义(P>0.05),腺癌患者Th17、Th17/Treg表达低于鳞癌患者,Treg表达高于鳞癌患者(P<0.05);Ⅰ~Ⅱ期患者NK细胞、Th1、Th1/Th2、Th17、Th17/Treg表达高于Ⅲ~Ⅳ期患者,Th2、Treg表达低于Ⅲ~Ⅳ期患者;有、无淋巴结转移患者NK细胞、Th17、Treg、Th17/Treg表达比较,差异有统计学意义(P<0.05);死亡组NK细胞、Th2、Th1/Th2、Th17、Th17/Treg表达低于生存组,Th1、Treg表达高于生存组(P<0.05)。结论外周血CD4^+T淋巴细胞亚群、NK杀伤细胞在NSCLC的发展与发生中发挥着重要的作用,在一定程度上可反映出患者病情变化,对了解患者免疫机制有重要的参考价值。
Objective To analyze the expression of peripheral blood CD4^+T lymphocyte subsets and natural killer cells(NK cells)inpatients with non-small cell lung cancer(NSCLC)and their clinical significance.Methods We selected 94 NSCLC patients admitted to our hospital as observation group and 89 healthy people who underwent physical examinations in our hospital during the same period as healthy group.The peripheral blood CD4^+T lymphocyte subsets and NK cells in different patients,different pathological types and different clinical stages,as well as patients with and without lymph node metastasis were compared.Results The expressions of NK cells,Th1,Th1/Th2,Th17,and Th17/Treg in the NSCLC group were significantly lower than those in the healthy group,while Th2 and Treg were significantly higher than those in the healthy group(P<0.05).There was no difference in the expression of NK cells,Th1,Th2 and Th1/Th2 between patients with adenocarcinoma and patients with squamous cell carcinoma(P>0.05).NK cells,Th1,Th1/Th2,Th17,and Th17/Treg of patients at stage Ⅰ~Ⅱ were higher than those of patients at stage Ⅰ~Ⅱ while NK cells,Th2,and Treg were lower than those of patients at stage Ⅲ~Ⅳ.The expressions of NK cells,Th17,Treg,and Th17/Treg between patients with and without lymph node metastasis were statistically different(P<0.05).There were significant differences in the expression of NK cells,Th1,Th2,Th1/Th2,Th17,Treg,and Th17/Treg between the death group and the survival group(P<0.05).Conclusion Peripheral blood CD4^+T lymphocyte subsets(Th1,Th2,Th17,Treg)and NK killer cells play an important role in the development and occurrence of NSCLC.It can reflect the changes of the patient’s condition to some extent,and has important reference value in understanding the immune mechanism of patients.
