摘要
目的探讨肾透明细胞癌(CCRCC)多排螺旋CT(MDCT)影像学表现与Fuhrman分级之间的相关性。方法回顾性分析芜湖市第二人民医院2015年1月至2019年1月共40例经过手术病理证实为CCRCC的CT图像资料,通过观察肿瘤的大小、形状、密度、包膜完整性以及CT增强特征等,与Fuhrman分级结果进行统计学分析。探究CCRCC的MDCT影像表现对预测其Fuhrman分级的应用价值。结果40例病人中,Fuhrman分级Ⅰ级2例,Ⅱ级30例,Ⅲ级7例,Ⅳ级1例,低级别组(Fuhrman Ⅰ~Ⅱ)与高级别组(Fuhrman Ⅲ~Ⅳ)肿瘤长径分别为(3.97±1.58)cm、(7.13±2.63)cm,形态规则的肿瘤分别为23例、2例,有完整包膜分别为20例、1例,增强扫描皮质期绝对强化值分别为(82.63±29.22)HU、(51.50±27.47)HU,廓清率分别为(43.27±12.65)%、(23.63±10.80)%,均差异有统计学意义(P<0.05)。在平扫密度上差异无统计学意义(P>0.05)。结论CCRCC的病灶长径、形状、有无完整包膜以及肿瘤强化特征等CT征象,对预测CCRCC的病理Fuhrman分级具有重要的临床意义,从而为临床治疗以及预后评估提供帮助。
Objective To investigate the correlation between multi-row spiral CT(MDCT)imaging findings and Fuhrman nucleus classification in renal clear cell carcinoma(CCRCC).Methods The CT images of 40 patients with CCRCC confirmed by surgery and pathology in The Second People’s Hospital of Wuhu from January 2015 to January 2019 were retrospectively analyzed.The tumor size,shape,density,envelope integrity and CT enhancement characteristics were observed.A statistical analysis was carried out with these characteristics and the results of Fuhrman nuclear grading to investigate the application value of MDCT imaging manifestations of renal clear cell carcinoma in predicting its Fuhrman nuclear classification.Results Of the 40 patients,2 cases were Fuhrman grade Ⅰ,30 cases were grade Ⅱ,7 cases were gradeⅢand 1 case was grade Ⅳ.The major diameter of the tumors in the low-grade group(Fuhrman Ⅰ-Ⅱ)and the high-grade group(Fuhrman Ⅲ-Ⅳ)was(3.97±1.58)cm and(7.13±2.63)cm,respectively.The regular tumors were 23 cases and 2 cases,respectively.The complete capsule was 20 cases and 1 case,respectively.The absolute enhancement values in the cortical phase were(82.63±29.22)HU and(51.50±27.47)HU,and the clearance rates were(43.27±12.65)% and(23.63±10.80)%,respectively,and the difference was significant(P<0.05).There was no significant difference in the density of plain scan(P>0.05).Conclusion CT signs based on the major diameter,shape,complete capsule and enhancement features of renal clear cell carcinoma have important clinical significance for predicting the pathological Fuhrman nuclear grading of CCRCC,thus providing assistance for clinical treatment and prognosis assessment.
