摘要
目的探讨个体化低剂量组合扫描方案对头颈部CT血管成像(CT Angiography,CTA)图像质量及辐射剂量的影响。方法选取首都医科大学附属北京天坛医院2018年11月至2019年10月收治的行头颈部CTA扫描的500例患者,采用随机数表法随机分为观察组和对照组,各250例。观察组采用个体化低剂量组合扫描(低对比剂、低管电流),对照组采用常规扫描(常规对比剂、常规管电流)。比较两组的图像质量和辐射剂量。结果两组的图像信号噪声比和对比噪声比差异无显著性(P>0.05)。与对照组相比,观察组的血管密度和肌肉密度更高,有效剂量、剂量长度乘积和容积CT剂量指数更低,差异有显著性(P<0.05)。结论个体化低剂量组合扫描方案对头颈部CTA扫描获取到的图像质量影响不大,但对辐射剂量的影响较为明显,可以减少患者检查过程中所受的辐射量,从而减轻辐射危害,有利于提高安全性。
Objective To explore the influence of individualized low-dose combined scanning program on the image quality and radiation dose of head and neck CTA.Method 500 patients with head and neck CTA scan admitted to our hospital from November 2018 to October 2019 were selected as the study subjects,and they were divided into observation group and control group with 250 cases.The observation group received individualized low-dose combined scanning(low contrast agent and low tube current),while the control group received conventional scanning(conventional contrast agent and conventional tube current).Compare two groups of general data,access to the image quality and radiation dose.Result In the comparison of SNR and CNR,there was no significant difference between the observation group and the control group(P>0.05).In the comparison of vascular density,muscle density and other image quality indicators,the observation group was higher than the control group,with statistically significant difference(P<0.05).In comparison of ED,DLP and CTDIvol,the observation group was all smaller than the control group,with statistically significant differences(P<0.05).Conclusion The application of individualized low-dose combined scanning scheme has little influence on the image quality obtained by head and neck CTA scan,but has a significant influence on the radiation dose,which can reduce the radiation amount received by patients during the examination process,and thereby reduce the harm caused by radiation.Therefore,it has higher rationality and personalized characteristics.
