血清脂蛋白相关磷脂酶A2与2型糖尿病视网膜病变的相关性研究 被引量:3

Correlationship between serum lipoprotein-associated phospholipase A2 and diabetic retinopathy in type 2 diabetes mellitus

摘要 目的探讨2型糖尿病(T2DM)患者的血清脂蛋白相关磷脂酶A2(Lp-PLA2)水平与2型糖尿病视网膜病变(DR)的相关性。方法选择2018年1月—2019年1月就诊于郑州大学第一附属医院内分泌科的T2DM患者共131例,根据眼底镜检查结果,将其分为T2DM伴DR组(DR组)及T2DM不伴DR组(DM组),并选择同期50例体检健康者作为对照(D组),收集各组研究对象的一般临床指标及生化指标,比较Lp-PLA2在各组间差异,并分析Lp-PLA2与血脂、血糖、DR的相关性。结果①DR、DM、D组研究对象性别、年龄、SBP、DBP、TCHO、TG、HDL-C、LDL-C的组间比较差异无统计学意义(P>0.05),DR组、DM组BMI高于D组,差异均有统计学意义(P<0.05),DR组与DM组BMI比较,差异无统计学意义(P>0.05);②三组间FBG、Lp-PLA2的比较差异均有统计学意义(P<0.05),DR组与DM组中病程、HbA1c的比较,差异无统计学意义(P>0.05),DR组、DM组HbA1c高于D组,差异均有统计学意义(P<0.05);③Lp-PLA2与TCHO、LDL-C、FBG、DR显著相关(P<0.05),其相关系数r分别为0.246、0.207、0.606、0.706。结论Lp-PLA2与TCHO、LDL-C等脂质代谢物质相关,其水平在T2DM合并DR患者中显著升高,并与DR呈显著正相关,提示Lp-PLA2可能参与DR的发生发展过程,其水平对预测2型糖尿病合并糖尿病视网膜病变有一定的价值,可能为2型糖尿病及糖尿病视网膜病变的治疗提供新的靶点和方向。 Objective To explore the relationship between serum lipoprotein-associated phospholipase A2(Lp-PLA2)level and diabetic retinopathy(DR)in patients sufferring type 2 diabetes mellitus(T2 DM).Methods A total of 131 patients with T2 DM who were admitted to our department from January 2018 to January 2019 were enrolled.According to the results of ophthalmoscopy,they were divided into T2 DM with DR group(DR group)and T2 DM without DR group(DM group).50 healthy controls(D group)were enrolled at the same time.The general clinical and biochemical parameters of each group were collected.The differences of Lp-PLA2 level among the groups were compared,and the correlation between Lp-PLA2 and blood lipids,blood glucose and DR was analyzed.Results There were no significant differences in gender,age,SBP,DBP,TCHO,TG,HDL-C and LDL-C among the DR,DM and D groups(P>0.05).The BMI of the DR group and the DM group were higher than those of the D group,and the differences were statistically significant(P<0.05).Wheras,no significant difference in BMI was observed between the DR group and the DM group(P>0.05).Lp-PLA2 and FBG were statistical different among the three groups(P<0.05).There were no significant differences in disease course and HbA1 c between the DR group and DM group(P>0.05).The HbA1 c of the DR group and the DM group were higher than those of the D group,and the differences were statistically significant(P<0.05).Lp-PLA2 was significantly correlated with TCHO,LDL-C,FBG,and DR,and their correlation coefficients r were 0.246,0.207,0.606 and 0.706 respectively.Conclusion Lp-PLA2 is associated with lipid metabolites such as TCHO and LDL-C.The level of Lp-PLA2 is notably elevated in patients who sufferring T2 DM with DR,and is significantly positively correlated with DR,suggesting that Lp-PLA2 may be involved in the development and progression of DR.It has certain value in predicting type 2 diabetes with diabetic retinopathy,and may provide new targets and directions for the treatment of type 2 diabetes and diabetic retinopathy.
