摘要
目的观察鼻内镜下微波治疗顽固性鼻出血联合术后术腔涂抹重组牛碱性成纤维细胞生长因子眼用凝胶的临床效果。方法便利选取2018年6月-2019年6月于该院顽固性鼻出血的患者90例,利用随机数字法分为两组,观察组(A组,共45例)和对照组(B组,共45例)。A组给予鼻内镜下微波治疗的同时,并与术后术腔涂抹重组牛碱性成纤维细胞生长因子眼用凝胶, B组给予鼻内镜下单纯微波治疗顽固性鼻出血。分析比较两组患者术中情况、术后疗效以及并发症情况。结果 A、B两组患者的鼻腔粘膜恢复时间不同,A组较B组恢复的更快,差异有统计学意义(χ~2=6.483,P<0.05);鼻腔恢复通气时间也不同,A组恢复通气的时间更快,差异有统计学意义(χ~2=5.681,P<0.05);A、B两组患者按照设定的治疗方案给予治疗后,观察组的总有效率为91.1%,明显高于对照组的77.8%,差异有统计学意义(χ~2=0.938,P<0.05);比较可知,两组患者的并发症的总发生率观察组低于对照组(χ~2=3.519,P=0.046);对照组的复发次数多于观察组,差异有统计学意义(χ~2=2.015,P=3.657),对照组的复发率为62.22%,明显高于观察组的26.67%,差异有统计学意义(χ~2=3.657,P=0.013)。结论鼻内镜下微波治疗顽固性鼻出血联合术后术腔涂抹重组牛碱性成纤维细胞生长因子眼用凝胶治疗临床疗效显著,能够明显降低复发率和并发症发生率,具有较好的临床效果,值得推广。
Objective To observe the clinical effect of endoscopic microwave treatment of intractable epistaxis combined with postoperative application of recombinant bovine basic fibroblast growth factor gel. Methods Ninety patients with intractable epistaxis in the hospital from June 2018 to June 2019 were convenient selected and divided into two groups by random number method. The observation group(group A, 45 cases) and the control group(group B, 45 cases). Group A was treated with microwave treatment under nasal endoscope, and recombinant bovine basic fibroblast growth factor eye gel was applied to the postoperative cavity. Group B was treated with microwave under nasal endoscope for intractable epistaxis. The intraoperative and postoperative effects and complications of the two groups were analyzed and compared. Results The recovery time of nasal mucosa in group A and B was different, the recovery time of nasal mucosa in group A was faster than that in group B,the difference was statistically significant(χ~2=6.483, P<0.05);the recovery time of nasal ventilation in group A was also different, and the recovery time of ventilation in group A was faster(χ~2=5.681, P<0.05);after the treatment of patients in group A and B according to the prescribed treatment plan, the total effective rate of the observation group was 91.1%, significantly higher than that of the control group 77.8%,the difference was statistically significant(χ~2=0.938, P<0.05). The total incidence of complications in the observation group was lower than that in the control group,the difference was statistically significant(χ~2=3.519, P=0.046). The recurrence frequency of the control group was more than that of the observation group, the difference was statistically significant (χ~2=2.015,P=3.657),The relapse rate of the control group was 62.22%, which was significantly higher than that of the observation group(26.67%),the difference was statistically significant (χ~2=3.657,P =0.013). Conclusion Endoscopic microwave treatment of intractable epistaxis combined with postoperative smear of recombinant bovine basic fibroblast growth factor gel has significant clinical effect, can significantly reduce the recurrence rate and incidence of complic ations, has a good clinical effect, and is worthy of promotion.
